Objective:1.To observe the clinical effect and safety of Yi Shen Hua Shi Granule in the treatment of early type 2 diabetic nephropathy2.To observe the effect of Yi Shen Hua Shi Granule on the level of oxidative stress and inflammatory factors,to explore the relationship between the therapeutic effect of Yi Shen Hua Shi Granule and the expression of NOX4 protein,and to preliminarily elaborate the possible mechanism of Yi Shen Hua Shi Granule.Methods:68 patients diagnosed as early type 2 diabetic nephropathy in our hospital were randomly divided into two groups:group A and group B.The patients in group a received conventional treatment,while those in group B received Yi Shen Hua Shi Granule on the basis of conventional treatment.24 hours urine and venous blood were collected before treatment,4 weeks and 8 weeks after treatment to detect FPG,HbA1c,TC,TG,SCR,BUN,ALT,AST,ROS,TNF-α,IL-6,Upro,UAER,8 OHdG and NOX4,and the results were statistically analyzed.Results:1.After 8 weeks of treatment,the levels of SCr,BUN,Upro and UAER decreased significantly(P<0.05),and the levels of EGFR increased significantly(P<0.05),and the levels of SCr,BUN,Upro and UAER in group B were significantly lower than those in group A(P<0.05),and the levels of EGFR were significantly higher than those in group A(P<0.05).2.The clinical efficacy evaluation of diabetic nephropathy in each group showed that the total effective rate of group B(94.12%)was higher than that of group A(85.29%),but the difference was not statistically significant(?~2=2.095,P=0.351>0.05).The evaluation of efficacy of TCM syndromes in each group showed that the total effective rate of group B(94.12%)was higher than that of group A(76.47%),but the difference was not statistically significant(?~2=4.323,P=0.115>0.05).3.There were no significant differences in blood glucose,blood lipid and liver function indexes between the two groups of patients.4.Compared with before treatment,after 4 weeks of treatment,the levels of ROS and 8OHdG in each group were significantly reduced(P<0.05).After 8 weeks of treatment,the levels of ROS and 8OHdG in each group were significantly lower than4 weeks of treatment(P<0.05),and group B was significantly lower than group A(P<0.05).5.Compared with before treatment,after 4 weeks of treatment,the levels of TNF-αand IL-6 in each group were significantly reduced(P<0.05).After 8 weeks of treatment,the levels of TNF-αand IL-6 in each group were significantly lower than 4weeks of treatment(P<0.05),and group B was significantly lower than group A(P<0.05).6.Compared with before treatment,after 4 weeks of treatment,the levels of NOX4 in each group were significantly reduced(P<0.05).After 8 weeks of treatment,the levels of NOX4 in each group were significantly lower than 4 weeks of treatment(P<0.05),and group B was significantly lower than group A(P<0.05).7.Linear regression analysis showed that the level of NOX4 was significantly positively correlated with the levels of ROS,8 OHdG,TNF-αand IL-6 in group B.Conclusion:1.Yi Shen Hua Shi Granule can significantly improve the renal function of patients with early type 2 diabetic nephropathy,to some extent,it can improve the clinical efficacy of patients.2.Yi Shen Hua Shi Granule can significantly inhibit the level of oxidative stress and inflammation in patients,and its mechanism may be achieved by regulating the expression of NOX4.
