| Objective:Osteitis is considered to be an important factor affecting the occurrence,development and prognosis of chronic sinusitis with nasal polyps(CRSwNP).Osteitis is related to many factors,but the main factor is the long-term stimulation of inflammation.Interleukin 22(IL-22)is a new type of proinflammatory cytokine discovered in recent years and may be involved in the pathogenesis of active rheumatoid arthritis(RA)and osteoarthritis(OA),Suggesting that IL-22 is associated with the onset of osteitis,but the correlation between IL-22 and osteitis in CRSwNP is not clear.Transforming growth factorβ1(TGF-β1)is a growth factor with multiple biological functions,and plays a key role in the formation,maintenance and repair of cartilage.IL-22 and TGF-β1 expression levels were detected in patients with chronic sinusitis with nasal polyps(CRSwNP)Correlation of its severity and correlation,to explore the relationship between IL-22 and osteitis in cases of eosinophilic(ECRSwNP)and non-eosinophilic(NECRSwNP)chronic sinusitis with nasal polyps.Method:1.Bone and mucosal specimens of the ethmoid sinus from the First Affiliated of Nanchang University and Xingguo County People’s Hospital inpatient surgery were collected from July 2019 to December 2019 according to the inclusion criteria.Chronic sinuses with osteitis before surgery were selected.Cases of inflammation with nasal polyps(O-CRSwNP)were the experimental group,30 cases,and cases of chronic sinusitis with nasal polyps(CRSwNP)without osteitis before surgery were the control group,30 cases,and normal nasal mucosa was the normal group,10 cases.According to the pathological results,the experimental group and the control group were distinguished into two subgroups of eosinophil type(ECRSwNP)and non-eosinophil type(NECRSwNP).The bone or mucous membranes obtained were tested by ELISA and qRT-PCR for IL-22 and TGF-β1 expression.2.Analysis of CT images of the sinuses of the patients before surgery:The thickness of the thickest part of the osteitis and the extent of the osteitis in the CT image of each sinus bone wall were measured.The severity of osteitis was diagnosed and evaluated by the GOSS score.3.The correlation between the expressions of IL-22 and TGF-β1 and osteitis in CRSwNP and their severity were compared,and the analysis was concluded.Result:1.Results of qRT-PCR:The IL-22 and TGF-β1 mRNA expressions in patients of the O-CRSwNP group were higher than those in the CRSwNP group and the control group(P<0.05);the IL-22 and TGF-β1 mRNA expressions in the ECRSwNP patients of O-CRSwNP group were higher than those in the CRSwNP group(P<0.05),and the IL-22 and TGF-β1 mRNA expressions in the NECRSwNP patients of O-CRSwNP group were higher than those in the CRSwNP group(P<0.05).2.Results of ELISA:The levels of IL-22 and TGF-β1 in patients of the O-CRSwNP group were higher than those in the CRSwNP group and the control group(P<0.05);the levels of IL-22 and TGF-β1 in the ECRSwNP patients of O-CRSwNP group were higher than those in the CRSwNP group(P<0.05),and the levels of IL-22 and TGF-β1 in the NECRSwNP patients of O-CRSwNP group were higher than those in the CRSwNP group(P<0.05).3.Correlation analysis results:The correlation between IL-22 and TGF-β1 in the CRSwNP group was not high,as well as O-CRSwNP group,NECRSwNP group,NECRSwNP group,O-NECRSwNP group and O-ECRSwNP;TGF-β1 and IL-22 in O-CRSwNP has a strong correlation with Goss,in the mean times,IL-22 also has strong association with the high goss of O-CRSwNP.Conclusion:1.The mRNA expressions and concentration of IL-22 and TGF-β1 in the O-CRSwNP group were significantly higher than those in the CRSwNP group.suggesting O-CRSwNP patients might suffer a high concentration of IL-22 and GF-β1 in samples.2.Correlation analysis between the concentrations of IL-22 and TGF-β1 and the Goss score between the CRSwNP group and the O-CRSwNP group.It was found that TGF-β1 in ECRSwNP showed a strong correlation with Goss,and TGF-β1 and IL-22in O-ECRSwNP showed a strong correlation with Goss,suggesting IL-22 might take a certain association with the occurrence of O-CRSwNP.3.The concentration of IL-22 and TGF-β1 in both of O-ECRSwNP and O-NECRSwNP group are significantly more than the concentration of CRSwNP group,but the results we found indicated no significant differences statistically in IL-22 and TGF-β1 between ECRSwNP and NECRSwNP group(p>0.05),suggesting concentration of IL-22 and TGF-β1 is not the main differential index in both ECRSwNP and NECRSwNP group.However,IL-22 and TGF-β1have very strong correlation in O-CRSwNP group(R~2 over 0.8),and the correlation between IL-22 and TGF-β1 is weak(R~2=0.0458),suggesting the infiltration of eosinophilic granulocyte,might be the inducement to increase the concentration of IL-22,and develop the rate of O-CRSwNP. |
