Objective: To detect the change of serum IL-34 in patients with active pulmonary tuberculosis before and after treatment,and to explore its clinical significance.Methods:Collecting clinical datas of 30 patients(APTB group)who were diagnosed active pulmonary tuberculosis and had 2 months of effective anti-tuberculosis treatment in the Respiratory and Critical Medicinea Tuberculosis Area of Affiliated Hospital of Zunyi Medical University during September 2019 to February 2020,including chest CT,sputum smear,ESR,C-reactive protein,etc.According to the chest CT results,APTB group was divided into cavitary group and non-cavitary as well as ≥3 fields group and <3 fields group,at the same time,30 cases of latent tuberculosis infection(LTBI group)and healthy control(HC group)were collected,tkaing 4ml peripheral venous blood of APTB group before and after treatment,LTBI group and HC group,using the ELISA method to detect serum IL-34 and IL-17.Comparing IL-34 and IL-17 levels in APTB group before and after treatment with those in LTBI group and HC group,to compare IL-34 and IL-17 levels in APTB group before and after treatment,comparing the levels of IL-34 and IL-17 between cavitary group and non-cavitary as well as ≥3 fields group and <3 fields group,to analyze the correlation between IL-34 and IL-17 as well as their correlation with the inflammatory indicators ESR and C-reactive protein.Data analyse were performed by using the SPSS software package version 19.0,All statistical analyzes were ɑ=0.05 leve for the test standard,P<0.05 was considered to be statistically significant.Results:1.The changes of serum IL-34 and IL-17 levels in APTB group before and after Treatment IL-34 and IL-17 levels in APTB group before treatment were significantly higher than those in LTBI group and HC group(P < 0.05);the levels of IL-34 in LTBI group was higher than that in HC group(P < 0.01);IL-17 levels in LTBI group was similar to that in HC group,there was no statistically significant(P>0.05);IL-34 and IL-17 levels in APTB group after 2 months of effective anti-tuberculosis treatment were lower than befor treatmen(P<0.05),but still were higher than those in LTBI group and HC group(P < 0.05);2.The comparison of serum IL-34 and IL-17 levels in cavitary group and non-cavitary group IL-34 and IL-17 levels in cavitary group were higher than those in non-cavitary group,the difference was statistically significant(P < 0.05);3.The comparison serum IL-34 and IL-17 levels in ≥3 fields group and <3 fields group the levels of IL-34 and IL-17 in ≥3 fields group were higher than those in <3fields group,the differrnce was statistically significant(P < 0.05);4.The correlation analysis between serum IL-17 and IL-34 as well as their correlation with ESR and C-reactive protein in APTB group before treatment the level of IL-34 was positive correlated with IL-17(r = 0.844,P<0.05);IL-34 was positively correlated with ESR(r =0.668 P<0.05)and C--reactive protein(r=0.596P<0.05);the level of IL-17 was positively correlated with ESR(r=0.566 P<0.05)and C-reactive protein(r =0.482 P<0.05).Conclusion: The level of serum IL-34 significantly increases in patients with active pulmonary tuberculosis,and is positively correlated with the severity of pulmonary lesions,and decreases after effective treatment,it suggests that IL-34 may involve in immune processes during the course of active pulmonary tuberculosis and become a new objective index to evaluate the severity and judge the efficacy of these patients.
