Background Stroke has become a major disease threatening human life and health because of its high incidence,high mortality and high disability rate.In the state of stroke,the main causes of brain injury are sudden hypoxia and ischemia,brain energy shortage,which lead to abnormal discharge of neurons,then nerve death in the core area and calcium overload in the ischemic penumbra after subsequent cerebral vascular opening,leading to further aggravation of brain injury.At present,there is no corresponding sustained brain protection strategy for the whole process of brain tissue injury.Although many studies have proved that anesthesia and sedation have protective effects on stroke,they are limited to meet the needs of surgery,and do not play the greatest protective role of anesthesia.Therefore,for patients with ischemic cerebrovascular disease undergoing interventional therapy,prolong the sedation time after operation,explore whether sedation after general anesthesia can protect the local ischemia-reperfusion injury,and provide the basis for the next study to propose the optimal anesthesia scheme of general anesthesia brain protection effect.Methods The degree of intracranial vascular stenosis in patients with cerebral infarction who were admitted to Renji Hospital Affiliated to Shanghai Jiaotong University School of medicine in 2018 was more than 70%(the degree of stenosis was judged by WASID method).15 patients with prolonged sedation for 0 hours were in the control group and 17 patients with prolonged sedation for 6 hours were in the experimental group.The basic body information and cerebral infarction were recorded and evaluated in detail in both groups before operation.The intraoperative anesthesia program was strictly controlled.After operation,the mortality,disability rate and brain function recovery(including NIHSS score scale and mRS scale)of the patients at 1 day and 1 or 3 months after operation were followed up and evaluated.Finally,the different prognosis of the two groups was analyze Results Compared with the patients without sedation and extubation(sedation 0 hours group),the NIHSS scores of the patients with sedation 6 hours group were improved one month and three months after operation,and the difference was statistically significant(P < 0.05).The mRS scores of the patients with sedation 3 months after operation were improved compared with those before operation,and the difference was statistically significant(P < 0.05).Conclusion Prolonging the sedation time after operation can improve the NIHSS score and the mRS score of 1 and 3 months after the interventional treatment of ischemic cerebrovascular disease,and exert the protective effect of anesthesia and sedation on the patients with the interventional Treatment of ischemic cerebrovascular disease.
