| Background: The physical and mental health of adolescences is the basis for the comprehensive development of our country and the key to whole nation.The health checkup work of middle school students is a solid guarantee for the healthy growth of students and a powerful hand to promote students’ physical and mental health.Through the regular physical examination of students every year,some important common diseases can be screened in advance.For example,poor vision,dental caries,overweight and obesity.The common adverse health outcomes of students caused by these adolescent health risk behavior factors include poor eyesight,dental caries,overweight and obesity,and high blood pressure.In recent years,the detection rate of overweight and obesity、poor eyesight and myopia has been increasing,and the main indicators of students’ physical fitness are keep declining.The purpose of this study was to investigate the current situation of health risk behavior factors and common adverse health outcomes of middle school students in Jiangsu province,which aimed at exploring the intrinsic relationship between the two.In addition,the TOPSIS combined with RSR was used to comprehensively evaluate the health monitoring of middle school students in Jiangsu Province in 2017.Therefore,relevant prevention measures and health promotion strategies are further proposed to provide scientific and effective protection for the physical and mental health of middle school students.Objectives:The study was to understand the health status and health risk behaviors of middle school students in Jiangsu Province through monitoring health condition and health risk behaviors of middle school students in Jiangsu Province.The TOPSIS combined with RSR was used to comprehensively evaluate the quality of health monitoring for middle school students in various cities in Jiangsu Province in 2017.Prevention strategies and measures were formulated for middle school students’ health risk behaviors.To provide scientific evidences for preventing and reducing the occurrence and development of common diseases among middle school students.Methods:1.Health monitoring status and risk factors analysis: The method of Multi-stage stratified random sampling was used to monitor the common diseases and health risk factors of middle school students in 13 cities in Jiangsu Province.1 urban area and 1 suburban county were randomly selected in each city.5 schools were randomly selected in urban area(including 2 junior high schools,2 high schools,1 vocational high school);3 schools were randomly selected in suburban county(including 2 junior high schools and 1 high school).Each grade is randomly selected from 1-2 classes,and each school monitors at least 50 students in each grade.Each student is required to complete a health checkup and an online questionnaire survey.The physical examination items include nutrition and growth indicators such as eyesight,dental caries,height,weight,and blood pressure.The questionnaire includes monitoring students’ unhealthy diet and some behaviors of weight controlling,lack of physical activity related behaviors,myopia related behaviors,psychological factors,substance abuse behaviors,and mental addiction behaviors,etc.The data were sorted by Excel 2003,and then the data was statistically described and analyzed using SPSS 22.0.2.Comprehensive evaluation and analysis of health monitoring: In the Excel,the TOPSIS combined with the RSR method was used to comprehensively evaluate 12 indicators such as height,weight,high blood pressure detection rate and caries rate in the health checkup of middle school students in Jiangsu Province in 2017,and SPSS 22.0 was used for data analysis.Results: 1、In this monitoring,18,668 questionnaires were used by middle school students in the province,and 18,382 questionnaires were actually completed.The effective response rate of the questionnaire was 98.5%.Among them,9401 boys and 8981 girls;the average age of middle school students is 14.7±1.8 years old.The detection rates of poor eyesight,dental caries,overweight and obesity and high blood pressure were 86.8%、34.6%、30.1% and 20.7% respectively in the examination of key common diseases of middle school students in the whole province.2、Univariate analysis of visual acuity and health risk behaviors among middle school students found that the detection rate of poor eyesight among middle school students had significant among the demographic characteristics of gender,urban and rural areas,school sections,and areas(P<0.05).Different degrees of poor visual acuity study found that different degrees of visual acuity in middle school students were different between the demographic characteristics of gender,urban and rural areas,school sections and areas(P<0.05).Univariate analysis of visual acuity found that whether to live in school,whether there is a phenomenon of uncomfortable Internet access,whether to meet the increase in online time,whether to know the consequences of online access,online,age group,nutritional status,the difference in the detection rate of poor eyesight among middle school students Statistical significance(P<0.05).Binary logistic regression analysis showed that Southern Jiangsu(OR=1.833,95%CI: 1.641-2.047),Central Jiangsu(OR=1.405,95%CI: 1.265-1.559),cities(OR=1.174,95%CI: 1.064-1.294),female(OR=1.821,95%CI: 1.664-1.992),high school(OR=2.408,95%CI: 2.069-2.803),and non-residence school(OR=1.256,95%CI: 1.138-1.385)were risk factors among middle school students(P<0.05).3、Univariate analysis of middle school students’ dental caries and health risk behavior factors showed that the detection rate of dental caries among middle school students had significant differences in the demographic characteristics of gender,school segment and areas(P<0.05).Binary logistic regression analysis showed that Southern Jiangsu(OR=2.059,95%CI: 1.906-2.224),Central Jiangsu(OR=1.593,95%CI: 1.474-1.722),female(OR=1.661,95%CI: 1.558-1.771),emaciation(OR=1.342,95%CI: 1.132-1.591),normal weight(OR=1.250,95%CI: 1.164-1.342)were the risk factors of dental caries among middle school students(P<0.05);Junior high school(OR=0.802,95%CI: 0.726-0.885)and senior school(OR=0.856,95%CI: 0.773-0.948)were protective factors of dental caries(P<0.05).4、Univariate analysis of overweight and obesity and health risk behaviors of middle school students shows that there are statistically significant differences in the detection rate of overweight and obesity among middle school students in gender,urban and rural areas,districts,whether to live in school,nationality,number of physical education classes,length of watching TV,whether to drink alcohol and age group(P<0.05).Binary logistic regression analysis showed that boys(OR=1.554,95%CI: 1.456-1.659),cities(OR=1.178,95%CI: 1.099-1.264),non-residential schools(OR=1.408,95%CI: 1.307-1.516),Huis(OR=4.004,95%CI: 1.908-8.399),and less than 1 hour of watching TV(OR=1.186,95%CI: 1.019-1.380),non-drinking alcohol(OR=1.074,95%CI: 1.001-1.152),the age group of < = 15(OR=1.121,95%CI: 1.045-1.202)were risk factors for overweight and obesity among middle school students(P<0.05);Southern Jiangsu(OR=0.854,95%CI: 0.788-0.925)and Central Jiangsu(OR=0.906,95%CI:0.837-0.981)were the protective factors of overweight and obesity in middle school students(P<0.05).5、Univariate analysis of high blood pressure and healthy risk behavior of middle school students shows that gender,urban and rural areas,school sections,districts,ethnic groups,the number of times of eating fruits in a week,the number of times of eating vegetables in a week,the number of days of drinking dairy products in a week,whether the lamps are LED lights,school Whether there was any face-to-face smoking and nutritional status had a statistically significant difference in the detection rate of high blood pressure among middle school students(P<0.05).Binary logistic regression analysis showed that boys(OR=1.091,95%CI: 1.012-1.176),rural(OR=1.446,95%CI: 1.334-1.567),Central Jiangsu(OR=1.243,95%CI:1.138-1.359),senior high school(OR=1.260,95%CI: 1.107-1.434),Hui(OR=3.017,95%CI: 1.295-7.028),eat fruit twice or more a week(OR=1.451,95%CI: 1.052-2.003),eat fruit once a week(OR=1.416,95%CI: 1.029-1.949),eat fruit less than once a week(OR=1.591,95%CI: 1.150-2.202),no one in the school smokes face-to-face(OR=1.189,95% CI: 1.078-1.312),overweight and obesity(OR=2.393,95% CI: 1.784-3.209),normal(OR=7.696,95% CI: 5.733-10.329)were risk factors for high blood pressure in middle school students(P<0.05).6、The comprehensive evaluation results of middle school students’ health monitoring showed that the physical health monitoring of middle school students in Xuzhou,Lianyungang and Suqian was excellent;Yancheng,Nanjing,Taizhou,Huai’an,Suzhou,Zhenjiang,Yangzhou and Nantong was good;Wuxi and Changzhou was moderate.Conclusions:The problem of key common diseases among middle school students was serious in Jiangsu Province,with high rates of poor eyesight,dental caries,overweight and obesity,and high blood pressure,especially poor eyesight among this group.The factors affecting the common diseases of middle school students are complex.Health,education and other relevant departments need to further intensify the publicity of health knowledge and concept according to the different characteristics of the middle school student population so as to help students to change their bad behavior habits.The combination of TOPSIS and RSR can further reflect the physical health monitoring of middle school students in 13 cities in Jiangsu Province.It was worthy of application in the comprehensive evaluation of students’ physical health monitoring,which can provide evidences for implementing,the physical health monitoring of students in the whole province. |
