| Purpose: To explore the treatment effect and safety for simultaneous or staged treatment of iliac vein compression syndrome with lower extremity varicose veins.Methods:The clinical data of 123 patients with iliac vein compression syndrome with lower extremity varicose veins,who were retrospectively analyzed of clinical data and postoperative follow-up.According to the procedure,the patients were divided into two groups: the same period treatment group(group A: one-time hospitalization for iliac vein stenosis and lower limbs varicose veins)and the staged treatment group(group B:first hospitalization for iliac vein stenosis,selective hospitalization for lower limbs varicose veins).There were 58 patients in group A,4 patients were lost to follow-up,and 54 patients were eventually included.There were 65 patients in group B,5 patients were lost to follow-up,and 10 patients did not take the next varicose operation because of the improvement of symptoms after iliac vein stent implantation,and finally 50 patients were included.Analyzed the patients’ hospitalization expenses,hospitalization time,bleeding,postoperative recurrence,stent patency rate and life quality.Results: The success rate of procedures were 100% in both groups.Group A and group B had no serious postoperative complications,the average hospitalization time were(9.26±2.15)days and(11.90±2.92)days respectively(P<0.001),the average hospitalization expenses were(4.25±0.93)ten thousand yuan and(4.86±0.82)ten thousand yuan respectively(P=0.001),the average intraoperative blood loss were(40.56±17.18)ml and(21.40±11.43)ml respectively(P<0.001),the postoperative ecchymosis were 38 cases and 25 cases respectively(P=0.03).Postoperative follow-up for 12 months.The VCSS scores of group A of preoperative and postoperative of 12 months were(6.52±3.70)and(1.04±0.67)respectively(P<0.001).The VCSS scores of group B of preoperative and postoperative of Iliac vein stent implantation 3 months were(6.38±3.17)and(4.04±2.19)respectively(P<0.001).The VCSS scores of group B of postoperative of Iliac vein stent implantation 3 months and postoperative of two operations 12 months were(4.04±2.19)and(1.22±0.71)respectively(P<0.001).The CIVIQ scores of group A of preoperative and postoperative of 12 months were(82.28±9.74)and(95.33±2.67)respectively(P<0.001).The CIVIQ scores of group B of preoperative and postoperative of Iliac vein stent implantation 3 months were(82.62±6.88)and(84.64±5.50)respectively(P=0.04).The CIVIQscores of group B of postoperative of Iliac vein stent implantation 3 months and postoperative of two operations 12 months were(84.64±5.50)and(94.84±2.11)respectively(P<0.001).In each group,there was 1 case of deep vein thrombosis of lower extremity during the period of hospitalization,which improved after treatment;the ulcer healing rate was100% in both groups;the stent patency rate was 100%,and there was no recurrence of varicose vein during the follow-up period.Conclusion: Simultaneous and staged treatment of iliac vein compression syndrome with lower extremity varicose veins is a safe and effective treatment with few complications and is worthy of clinical promotion. |
