Objective Through the retrospective analysis of the case data,to understand the risk factors causing spontaneous rupture of hepatocellular carcinoma,and refer to relevant literature to explore its impact mechanism.Provide possibilities for early clinical intervention.Methods In this study,51 patients with spontaneous rupture of hepatocellular carcinoma admitted to the General Hospital of Ningxia Medical University from 2012 to2019 were selected as the study group,and the patients without rupture of hepatocellular carcinoma occurred at the same time were randomly selected as the control group for retrospective analysis.The age,gender,tumor status(size,number,location,etc.),laboratory test results(AFP level,Hemoglobin,transaminase,coagulation function,etc.)and the combination of factors such as hepatitis,cirrhosis and hypertension were statistically analyzed.SPSS 26.0 software was used to performχ2 test or Fisher’s exact test on the count data,the t test was used for the normal distribution data,and the rank sum test was used for the skewed data for single factor analysis.Then the logistic regression model was established,and the selection method of variables was stepwise regression(ain=0.05,aout=0.1),with p<0.05 as the difference was statistically significant,so as to determine the independent risk factors leading to spontaneous rupture of hepatocellular carcinoma.Results The univariate analysis results of the two groups showed:gender,hemoglobin(HGB),total bilirubin(TBIL),aspartate aminotransferase(AST),alanine aminotransferase(ALT),γ-glutamyltransferase(GGT),prothrombin time(PT),international normalized ratio(INR),activated partial thromboplastin time(APTT),tumor size,alpha-fetoprotein(AFP),portal vein tumor thrombosis(PVTT),Child-Pugh score were statistically significant.Multivariate logistic regression analysis showed that the differences in TBIL,tumor size,and PVTT were statistically significant(p<0.05),and were independent risk factors for spontaneous rupture of hepatocellular carcinoma.Conclusion TBIL levels,tumor size,and PVTT might be independent risk factors for spontaneous rupture of hepatocellular carcinoma.
