An Epidemiological Study On Periodontal Status Of 65-74 Year Old People In Shanxi Province

Objective:To understand the periodontal status,oral knowledge,information and behavior of65-74 year old people in Shanxi Province,study the epidemiological characteristics of periodontitis,complete disease mapping,and analyze its influencing factors,so as to provide baseline data for promoting the oral health of the elderly and scientific basis for relevant departments to formulate oral health policiesMethods:This study adopts the method of multistage random sampling such as capacity,begin from 11 cities in Shanxi Province according to its distribution feature extraction of five cities,and USES the PPS sampling from these five cities each extraction area in a county,a total of 10 counties(area),to the 10 counties,65-74 year old the old cross-sectional survey,periodontal status of the samples were 900,gender parity,parity in urban and rural areas.The data collection of the survey consists of three parts: the first part is the questionnaire survey,which is designed according to the fourth national oral health epidemiological survey and conducted in a face-to-face manner on the sample population,so as to understand the basic situation,oral knowledge,information,behavior and general health of the elderly.The second part is physical examination and oral examination.Physical examination included measurements of height,weight and blood pressure,and oral examination included bleeding gums,calculus,depth of periodontal pockets,and loss of attachment.The third part is to collect the physical geographical andsocial economic data of the survey area by referring to shanxi statistical yearbook(2019)and data network of China meteorological administration,and conduct discrete processing of the collected data.Establish database after checking and cleaning the survey data.Chi-square test or non-parametric test was used to compare groups.The influencing factors were analyzed by Logistic regression analysis,and the spatial Statistics based on the prevalence of periodontitis were conducted by bayesian spatial hierarchical model and q-statistics.Results:This study should investigate 900 people,and actually investigate 900 people,among which the effective data is 898,and the effective response rate is 99.8%.The correct rate of oral knowledge among the surveyed population was 47.4%,and the correct rate of oral positive attitude was 79.1%,which was higher in urban areas than in rural areas(P < 0.05).5.8% of the elderly had good brushing habits,and the usage rate of floss and interdental brushes was 1.7% and 3.3%,respectively.In the survey population,70.5% of the elderly had dental experience,of which only 1.4% were for prevention purposes.In the people who have dental experience,the rate of dental visits in the past year is 23.1%,the number of teeth cleaning only accounted for 6.8%,for the people who have not seen a tooth in the past year,55.2% and 28.9% respectively believe that their teeth are no problem and dental disease is not serious.In the investigated population,the detection rates of gingival bleeding,calculus,periodontal bag and attachment loss were 83.2%,91.1%,57.4% and 76.4%,respectively.The prevalence of periodontitis was 69.8%.There was no statistical difference in the prevalence of periodontitis between urban and rural areas,but there was significant spatial differentiation(q=0.43,P < 0.05).Logistic regression analysis showed that education level,frequency of brushing teeth,oral health cognition,smoking,hypertension and diabetes were all influential factors for periodontitis among the elderly in Shanxi Province.The results ofgeographical detector showed that the prevalence of periodontitis in the elderly in Shanxi Province was higher in the north and lower in the south,and higher in the west and lower in the east.Average annual temperature,relative humidity,altitude and per capita GDP are the main influencing factors.Conclusion:1.The elderly aged from 65 to 74 in Shanxi Province have a positive attitude towards oral cavity,but their knowledge of oral health is limited.Medical treatment behavior is not positive,not to form the concept of prevention;Good oral health care habits have not been formed,and oral health behavior is single.2.The periodontal condition of 65-74 year old people in Shanxi Province is not good,the detection rate of all indicators and the prevalence rate of periodontitis are higher than the national average,and there is a spatial differentiation,which is manifested as the geographical trend of high in north and low in south,high in west and low in east.3.The prevalence rate of periodontitis among 65-74 year old people in Shanxi Province is affected by a variety of factors,including education level,oral health cognition,frequency of brushing,smoking,hypertension,diabetes,annual average temperature,altitude,relative humidity,and per capita GDP.
