Objective:By measuring the expression level of Galectin-3(Gal-3)in patients with persistent atrial fibrillation(PsAF),we explored Gal-3 alone and combined with left atrial diameter,LAD)Predictive value of recurrence after radiofrequency ablation in patients with persistent AF.Methods:A total of 52 healthy people from December 2017 to May 2019 in our hospital medical examination center were selected as the control group.The patients who were hospitalized during the same period and met the 2016 ESC Atrial Fibrillation Management Guidelines for continuous atrial fibrillation diagnosis and performed the first RFA Fifty cases were used as research objects.Patients with persistent AF were divided into non-relapsed group and relapsed group according to whether the patient relapsed after radiofrequency ablation.Subjects in each group were recorded:gender,age,body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),fasting blood glucose fasting plasma glucose(FPG),triglyceride(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL),high density lipoprotein cholesterol(HDL),smoking history,drinking history,medication history(β-blockers,amiodarone,anticoagulant drugs),duration of AF,left atrial inner diameter(LAD),left ventricular end-diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),high sensitive C-reactive protein(hs-CRP)B-type natriuretic titanium(BNP),Gal-3 serum levels.Comparing the basic information and the difference of serum Gal-3 level between healthy control group and persistent AF group,non-relapsed group and relapsed group.And then using Pearson correlation analysis to explore the relationship between Gal-3level and LAD,duration of AF The relationship between the indicators.Logistic multivariate regression analysis was used to explore the risk factors of recurrence after radiofrequency ablation in patients with persistent atrial fibrillation,and the receiver operating curve(ROC curve)was used to analyze serum Gal-3 levels alone and combined with LAD Predictive value of recurrence after radiofrequency ablation in patients with persistent AF,and survival analysis of recurrence after radiofrequency ablation in patients with persistent AF using Kaplan–Meier method.Results:1.There were no significant differences in gender ratio,smoking history,drinking history,BMI,SBP,DBP,LVEF,LVEDD,FPG,TG,TC,LDL,HDL between the healthy control group and persistent atrial fibrillation group(all P> 0.05),and there were significant differences in age,heart rate,LAD,hs-CRP,and BNP between the two groups(all P <0.05);The differences of gender ratio,smoking history,drinking history medication history(β-blockers,amiodarone,anticoagulants),BMI,SBP,DBP,LVEDD,LVEF,FPG,TG,TC,LDL,HDL,hs-CRP and BNP of the patients in the non-relapsed group and the relapsed group were no statistical significance(all P> 0.05).There was a statistically significant difference in age,heart rate,and LAD between the two groups(all P <0.05).2.The serum level of Gal-3 in persistent atrial fibrillation group was significantly higher than that in the healthy control group,the difference was statistically significant(P <0.05);the serum level of Gal-3 in the recurrence group was significantly higher than that in the non-recurrent group,the difference was statistically significant(P <0.05).3.Pearson correlation analysis showed that there was a significant positive correlation between serum Gal-3 levels and LAD(r> 0,P <0.05);there was a significant positivecorrelation between serum Gal-3 levels and the duration of atrial fibrillation(r> 0,P<0.05).4.Logistic multivariate regression analysis showed that serum Gal-3 levels and LAD were independent risk factors for recurrence after radiofrequency ablation in patients with persistent AF.5.ROC curve analysis results show that the best cut-off value of Gal-3 alone for predicting persistent AF recurrence is 5.42 ng / ml,and its accuracy is 81.2%;the best cut-off value of LAD alone for predicting persistent AF recurrence is 34.4mm Its accuracy is 86.3%;the accuracy of Gal-3 combined with LAD in predicting recurrence after radiofrequency ablation in patients with persistent atrial fibrillation is 92.3%,indicating that the combined predictive value of the two indicators is superior to the predictive value alone.6.Kaplan–Meier survival analysis showed that as the follow-up time increased,the non-recurrence rate in the Gal-3> 5.42 ng / ml group was significantly lower than that in the Gal-3 ≤ 5.42 ng / ml group(P <0.05);also,the non-recurrence rate in the LAD>34.4mm group was significantly lower than that in the LAD≤34.4mm group(P <0.05).Conclusion:1.Serum Gal-3 level is associated with recurrence after radiofrequency ablation in patients with persistent atrial fibrillation.2.Serum Gal-3 level is a risk factor for recurrence after radiofrequency ablation in patients with persistent atrial fibrillation.3.Serum gal-3 level can provide positive prediction value for recurrence after rf ablation in patients with persistent atrial fibrillation,and the prediction value of serum gal-3 level combined with LAD is greater.The determination of serum gal-3 levels may be useful in determining appropriate candidates and timing for radiofrequency ablation in patients with atrial fibrillation.
