Current Situation And Countermeasures Of County Medical Community Operation In Fujian Province

Objective:Under the background of the country’s active implementation of the construction of medical complexes,this study is a research site for the typical pilot areas of the three county medical communities in Fujian Province,summarizing its operational status,operational effects and obstacles,and exploring the influence of county medical institutions.The relevant factors of the construction and development of the community,and proposed measures to promote the sustainable development of the county medical community in Fujian Province.Methods: This study used a combination of qualitative and quantitative methods to study the current status of the operation of county medical communities in Fujian Province.First,literature research.Collect relevant literatures on the theoretical knowledge of medical communities at home and abroad,and the reform practices of primary health care in various regions.Second,the questionnaire survey.Including the medical organization structure,health workforce,medical expenses,business income,two-way referral and other aspects of the information,as well as the medical staff in the medical community to investigate the satisfaction.Third,interviews with informed individuals.Through semi-structured interviews,interviews with relevant department leaders,principals of medical associations,and heads of township health centers were conducted to understand the effectiveness and operational obstacles of the implementation of the medical community and the opinions and suggestions on the construction of medical institutions.Fourth,statistical analysis.Statistical analysis was carried out with SPSS20.0.Descriptive analysis was used to describe the current status of county medical community operation.One-way analysis of variance,t-test and sputum test were used to analyze the distribution differences of medical staff satisfaction among different factors.Multiple linear regression analysis was used to analyze the factors affecting the satisfaction of medical staff.Results:(1)Operational objectives: The three county medical communities all aim to promote the balance and sinking of medical resources and promote graded diagnosis and treatment as the main goal.The specific goal is to achieve a 90% visit rate in the county.(2)Organizational structure: The medical community integrates the county and the third-level hospitals to establish the general hospital,implements the president’s responsibility system,implements direct management of the primary medical institutions,and integrates and sets up functional departments.(3)Management system: The medical community implements unified management of the people,finances,materials and information of the member units,and establishes a close-knit medical association with shared responsibility,benefit sharing and complementary advantages.(4)Operational effects: the medical community infrastructure has been improved,the overall medical qualifications and professional titles are low,the medical expenses are generally declining,the income and expenditure of medical institutions are generally on the rise,the number of two-way referrals is increasing year by year,and grassroots support In an orderly manner,the overall satisfaction of medical staff is high.Through interviews,the main problems of medical community operation are obtained: low understanding of the medical community policy,poor communication of information within the medical community,shortage of health talents,and insufficient financial input.Conclusion: The initial results of the operation of the county medical community are mainly reflected in the improvement of the county’s vertical medical and health service system,the improvement of primary medical service capacity,and the stable economic operation of the medical community.However,there are fewer health human resources in operation,the compensation incentives and assessment mechanisms within the medical community have not been established,the two-way referral channels are not smooth enough,the level of social and economic development in mountain areas restricts the construction of medical institutions,and the publicity of medical community-related policies is not in place.problem.Based on this,the government will increase investment in the medical community,formulate corresponding preferential policies,strengthen the construction of the health human resources team,promote the construction of information technology,realize the information exchange within the medical community,encourage doctors to practice more,and improve the medical community.Suggested measures for the construction of the salary performance system and the improvement of the two-way referral incentive system.
