Analysis And Thinking Of Clinical Characteristics Of Intrahepatic Cholangiocarcinoma

Objective The clinical characteristics of patients with intrahepatic cholangiocarcinoma were retrospectively analyzed to facilitate early diagnosis and provide the possibility for early clinical intervention.Take corresponding treatment methods for different patients to prolong the overall survival time of patients.Methods A retrospective analysis was performed on 49 patients with intrahepatic cholangiocarcinoma treated at the General Hospital of Ningxia Medical University from January 2011 to December 2018.In addition,49 patients with primary hepatocellular carcinoma treated at the Ningxia Medical University General Hospital during the same period were randomly selected.Patients with primary hepatocellular carcinoma served as a control group.The patient’s age,gender,laboratory test results（bilirubin,transaminase,ALP,GGT,CA19-9,CEA,AFP,etc.）,tumor size,number of tumors,and whether HBV-related cirrhosis was statistically analyzed.SPSS 26.0 software was used to performχ2 test or Fisher’s exact test on the count data,the t test was used for the normal distribution data,and the rank sum test was used for the skewed data for single factor analysis.Then a logistic regression model was established.The method of variable selection was stepwise regression(?_in（28）0.05,?_out（28）0.1),P<0.05 was considered statistically significant.The clinical characteristics of intrahepatic cholangiocarcinoma were clarified.To clarify the clinical characteristics of intrahepatic cholangiocarcinoma.Results The main clinical manifestations of 49 ICC patients were intermittent epigastric dull pain with or without jaundice.The univariate analysis results showed:gender,alkaline phosphatase（ALP）,γ-glutamine transferase（GGT）,carcinoembryonic antigen（CEA）,CA19-9,CA12-5,prothrombin time（PT）,the activated partial prothrombin time（APTT）,prothrombin international normalized ratio（INR）were statistically significant.Multivariate logistic regression analysis showed that the differences in CA19-9,alpha-fetoprotein（AFP）were statistically significant（P<0.05）.This can be used as a basis for judging the clinical characteristics of intrahepatic cholangiocarcinoma.Conclusion 1.ICC has a hidden onset,no specific clinical symptoms,high malignancy,and aggressiveness.It has the characteristics of high CA19-9 positive rate and low AFP positive rate.These can provide some judgement for clinical diagnosis;2.Radical surgery is the preferred treatment for this disease,which can prolong the prognosis of patients,but patients with advanced disease still have high recurrence and high metastasis after surgery.3.Because ICC patients have the characteristics of occult intraperitoneal metastases,diagnostic laparoscopy is used in the treatment of this disease,which can save patients who have missed surgery from laparotomy biopsy.
