Objective:To study the changes of Neprilysin(NEP)levels in patients with different grades of cardiac function and to evaluate its correlation.Methods:From September 2018 to April 2019,58 patients with heart failure group(HFG)who underwent four cardiopulmonary tests and color Doppler echocardiography(CDFI)in the Department of Cardiology of the Second Hospital of Shanxi Medical University were selected as the control group.At the same time,26 healthy people who underwent health examination in our hospital at the same time were selected as the control group.CG).Serum samples were collected within 24 hours after admission and stored in refrigerator at-80 C.Serum enkephalin levels were detected by selective ELISA.Left ventricular ejection fraction(LVEF),left atrial meridian(LA)and left ventricular end-diastolic diameter(Left ventricular end-diastolic diameter)were measured by BNP and color Doppler echocardiography.Ntricular end diastolic(LVED)was used for correlation analysis.Results:1.the level of serum NEP(435.91 ± 86.12 pg / ml)in the heart failure group was significantly lower than that in the control group(555.99 ± 116.75 pg / ml),and the difference was statistically significant(t =-4.70,P = 0.001 < 0.01);2.the level of NEP in the heart failure group was as follows: the level of cardiac function II Group(475.38 ± 92.97 pg / ml);the level of cardiac function III group(418.45 ± 67.32 pg / ml);the level of cardiac function IV Group(409.39 ± 72.93 pg /ml),In the heart failure group,there was a significant difference among the three groups of serum NEP(F = 3.757,P = 0.030).There was a significant difference between the two groups(P = 0.037).There was a significant difference between the two groups(P =0.036).There was no significant difference between the three groups(P = 0.731);the level of NEP in other groups was not statistically significant(P > 0.05).3.La and LVEDd in heart failure group were significantly higher than those in normal group(t = 4.35,P = 0.001 < 0.01;t = 2.26,P = 0.026 < 0.05,respectively);4.NEP levels of different ejection fraction levels in heart failure group were:ejection fraction reduction group:(443.58 ± 73.18 pg / ml);ejection fraction median group:(456.59 ± 73.98 pg / ml);ejection fraction retention group:(414.21 ± 99.6);5.There was no significant correlation between BNP,EF,La,LVEDd and NEP(P >0.05).Conclusion:1.With the development of chronic heart failure,the level of serum NEP is lower than the normal level,which may be closely related to the compensatory consumption of NEP in the body of patients with chronic heart failure,so as to slow down the degree of heart failure;2.NEP may play an important role in the progress of chronic heart failure;however,their levels do not directly reflect the heart function grading of heart failure,nor do they have LVEF,La and LVEDd;3.The level of serum NEP may be a new laboratory evidence for the diagnosis of chronic heart failure,but it can not clearly predict the severity of heart failure. |
