Objective:To explore the effect of multi-mode analgesia guided by ropivacaine incision infiltration on analgesia and rehabilitation after ERAS(enhanced recovery after surgery)laparoscopic radical resection of rectal cancer.Methods :127 patients undergoing laparoscopic radical resection of rectal cancer in our hospital were randomly divided into two groups.Analgesic pump analgesia group(group A,n = 65)Postoperative analgesia methods: flurbiprofen ester 200 mg + tropisetron 20 mg + dizosin 10 mg + sufentanil 0.2 mg + saline configuration into 100 ml liquid analgesic pump(1 ml per hour);Multimodal analgesia group(group B,n = 62)Postoperative analgesia methods: flurbiprofen ester 50 mg + saline 40 ml intravenous injection;Oxycodone 1 tablet oral;Methylprednisolone 20 mg intravenous injection;Drops;Ropivacaine 5 ml subcutaneous porous tube incision infiltration.ERAS mode management was performed during perioperative period.Visual Analogue Score(VAS)was observed and recorded at 9:00 and 17:00 a.m.on the 1st,2nd and 3rd day after the operation.The first exhaust,the time of removal of urinary catheter and abdominal drainage catheter,the time of discharge and the actual time of discharge were recorded.The daily distance from bed and the amount of food taken by the patients on the 1st,2nd and 3rd days after the operation were recorded.Inflammation index on the first day and the first and third days after operation,gastrointestinal peristalsis on the second day after operation,incidence of adverse reactions,anxiety score and sleep quality were recorded.Results :The VAS of rest and exercise in group B was significantly lower than that in group A(P < 0.05).The daily activity distance of group A was significantly longer(P < 0.05),the time of exhaust,catheter removal and discharge was significantly shorter(P < 0.05),the inflammation index was lower(P < 0.05),the recovery of gastrointestinal peristalsis was faster(P < 0.05),and the incidence of adverse reactions of nausea and vomiting was significantly reduced(P < 0.05).Conclusion:Multimodal analgesia guided by ropivacaine incision infiltration after ERAS laparoscopic radical resection of rectal cancer can provide patients with good analgesic effect,alleviate inflammation,accelerate recovery of gastrointestinal function,alleviate patients’ anxiety and reduce hospitalization days. |
