Objective:To evaluate the feasibility, efficacy and safety of TIPSS in patients with portal vein thrombosis with cavernous transformation. Methods:The literatures were retrieved from PubMed,Cochrane library,Embase,Web of Science,CNKI,wanfang and weip databases, relevant data were extracted, and meta analysis was performed on the data using RevMan 5.1 software. Results:According to inclusion and exclusion criteria,9 studies were included finally, and a total of 188 patients, all of them were patients with portal vein thrombosis with cavernous transformation, received TIPSS treatment. The success rate of meta-analysis was 77%(95%CI=63-87%).I~2=67%,P=0.0005);The incidence of major complications was 22%(95%CI=11%-40%).I~2=59%,P=0.004);The Porto-systemic pressure gradient before and after surgery WMD=12.10(95%CI=11.86-14.52).P<0.00001);The combined rate of rebleeding was 12%(95%CI=7%-21%).I~2=0%,P<0.00001);The incidence of postoperative hepatic encephalopathy was 17%(95%CI=11%-25%).I~2=0%,P<0.00001);The stent patency rate in 12 months was 81%(95%CI=73-86%).I~2=0%,P<0.00001);The survival rate at 12 months was combined to 89%(95%CI=81-94%).I~2=16%,P<0.00001).Conclusion:We think TIPSS is feasible, effective and safe in treating patients with portal vein thrombosis with cavernous transformation.
