Objective:To explore the correlation between serum prealbumin(PA)and complications in patients accepted radical resection of lung cancer,and to provide theoretical support for reducing the incidence of complications after lung cancer surgery.Methods:According to the inclusion criteria and exclusion criteria,collected268 patients accepted radical resection of lung cancer with thoracoscope in July 2014-June 2019 in The First Hospital Affiliated to Chongqing Medical University thoracic surgery as the research object.Depending on whether the patients occur complications,it can be divided into complications group and the control group.Depending on the type of complication,the complication group was divided into pulmomary infection group,atelectasis group...and so on.PA of each patient was collected before surgery and 2-5 days after surgery.At the same time,clinical data such as age,sex,smoking index,BMI,preoperative complications and drainage time were collected.And statistical analysis was conducted between the PA level of each complication group and the control group.Then,ROC curve analysis was performed on the positiveresult group to find out the optimal critical value of PA level between each group and the control group.Finally,Logistic regression analysis was used to determine the correlation of postoperative PA to the corresponding complications in the positive group.Results:Statistical analysis showed that there was no significant difference in the level of PA in each group before operation.On the second to fifth day after the operation,there was a statistically significant difference in PA level between the pulmonary infection group((?)±S =105.9±29.5 mg/L,p<0.001)、thoracic drainage increasing group((?)±S =100.7±31.5 mg/L,p<0.05)and the control group((?)±S =139.2±30.8 mg/L).While there was no statistically significant difference between the other groups and the control group(P>0.05).After ROC curve analysis,the optimal critical value of PA between the pulmonary infection group and the control group was 119.5mg/L.At this time,the sensitivity and specificity of this index in predicting pulmonary infection were 67.5% and 74.7%.The optimal critical value of PA between thoracic drainage increasing group and control group was 121.5mg/L.At this time,the index predicted the sensitivity of thoracic drainage increasing group : 77.8%,and the specificity: 72.5%.Logistic regression analysis showed that PA < 120mg/L(P<0.001,OR=0.122)after operation was closely related to the occurrence of postoperative pulmonary infection,and PA < 120mg/L(p=0.023,OR=0.883)was closely related to the increasing of postoperative chest drainage.Conclusion:Serum prealbumin will decrease after surgery,and patients whose serum prealbumin less than 120 mg/L after surgery will get a greater risk of pulmonary infection and thoracic drainage increasing than other patients.Therefore,these patients should be carefully observed and early clinical intervention should be carried out if necessary.
