Objective: Although statins is used properly, the residual cardiovascular risk remains high.Foreign guidelines advocate supplying omega-3 fatty acids to reduce the occurrence of cardiovascular events, but the results of a large number of tests on omega-3 fatty acids are contradictory. This article compares the efficacy of statin combined with omega-3 fatty acids versus statin alone in reducing cardiovascular risk. Methods: A systematic electronic literature search for randomized controlled studies(RCT)of statin combined with omega-3 fatty acids versus statin alone in reducing cardiovascular risk in Pubmed,Embase and The Cochrane Library. The data of major adverse cardiovascular events(MACE),non-fatal myocardial infarction, cardiovascular death, stroke, coronary revascularization were collected, and analyzed by RevMan5.3.Results: 4 RCTs were included, with a total of 27,300 patients,of which 13,655 were in omega-3 fatty acid + statins group and 13,645 were in statins group. The results showed that omega-3 fatty acid + statins(7.3%)had a lower incidence of MACE than statins alone(9.3%)(RR = 0.79,95%CI(0.73-0.85),p <0.00001).Among the patients with secondary prevention, the incidence of MACE was 15.1% in the omega-3 fatty acid +statin group and 19.7% in the statin group(RR = 0.77,95% CI(0.70-0.83),p <0.00001).In addition,non-fatal myocardial infarction(RR = 0.73,95%CI(0.63-0.84),p <0.0001),coronary revascularization(RR = 0.75,95% CI(0.67-0.83),p <0.00001)and cardiovascular death(RR = 0.83,95% CI(0.69-0.99),p = 0.04)in the omega-3 fatty acid + statins group were less than in the statins group. However, the incidence of stroke(RR = 0.84,95% CI(0.60-1.18),p = 0.31)and MACE of the primary prevention(RR =0.86,95% CI(0.73-1.01),p = 0.07)had no significant difference in both groups. Conclusion: Omega-3 fatty acids can reduce cardiovascular risk furtherly based on the use of statins.
