Objective: Diabetes is a metabolic disease mainly with hyperglycemia,and diabetic kidney disease(DKD)is one of its main microvascular complications.The occurrence of DKD is affected by many factors,among which the blood glucose fluctuations have attracted much attention.In this study,the continuous glucose monitoring system was applied to quantify time in range(TIR)and explore its correlation with the level of urinary albumin.Methods: A total of 59 patients with type 2 diabetes were selected,those who were hospitalized in the Endocrinology Department of Hebei General Hospital from October 2019 to January 2020.They were divided into normal-urine albumin group(NUA)and urine albumin group(UA)based on 24-hour urine microalbumin(UMA)levels.General data were recorded for each patient,and homocysteine of blood lipid and blood albumin was detected for 24 hours.Abbot assisted instant sensing blood glucose meter was used to monitor the blood glucose of the patient for 14 days,and the results were summarized.The correlation factors affecting urinary albumin level were analyzed,and the correlation between TIR and urinary albumin level was discussed.Results:1.Comparison of basic clinical data between NUA group and UA groupA total of 59 patients with type 2 diabetes were enrolled,which included 13 in the UA group and 46 in the NUA group.The abnormal rate of urine albumin levels was 22.03%.FPG and Hb A1 c in UA group were lower than that in NUA group,and TG was higher than that in NUA group.The differences were statistically significant(P <0.05).In this study,homocysteine,age,systolic blood pressure,diastolic blood pressure,body mass index,serum creatinine uric acid,glomerular filtration rate,total cholesterol,high-density lipoprotein-cholesterol,low-density lipoprotein-cholesterol,total cholesterol/high-density lipoprotein-cholesterol were compared between the UA group and the NUA group,and the results showed no significant difference between the two groups(P >0.05).2.Comparison of TIR between NUA group and UA groupTIR in UA group was lower than that in NUA group,and the difference was statistically significant(P <0.05).3.Comparison of homocysteine(Hcy)before and after treatmentIn this study,the differences of Hcy before and after hospitalization in 48 patients with type 2 diabetes mellitus were compared.The results indicated that Hcy after treatment was lower than that before treatment,and the difference was statistically significant(P<0.05).4.Analysis of the effect of TIR and other factors on urine albumin levelsLogistic binary regression analysis showed that TIR was a independent protective factor for maintaining normal levels of urinary albumin.After FPG,Hb A1 c and TG were controlled at the same time,TIR is still a protective factor to maintain normal urine albumin levels.Conclusion:1.TIR in the UA group was significantly lower than that in the NUA group,indicating that the longer the period of good blood glucose controlled,the less likely the occurrence of urinary albumin,which further confirming the importance of smooth glucose reduction.2.TG in the UA group was higher than that in NUA group,suggesting that TG might be related to the production of urinary albumin.Well lipid-lowering treatment would be beneficial for the prevention and treatment of DKD.3.The level of Hcy decreased by the blood glucose was controlled gradually after hospitalization,suggesting that controlling blood glucose might reduce complications by reducing Hcy level.4.Logistic regression analysis showed that TIR is a protective factor to maintain normal-urine albumin levels. |
