Objective: Objective to analyze the risk factors of lymph node metastasis in early stage lung cancer and to provide reference for lymph node dissection smaller than 3cm.Methods: Lung cancer resection and systematic lymph node dissection were performed on 725 patients with lung cancer under 3cm to analyze the metastasis pattern of lymph nodes.Results: Lymph nodes metastasis rate in each group were affected by visceral pleural invasion(Group2、3、4、5 and 6,P=0.000;Group7,P=0.038;Group10,P=0.002;Group8 and 9,P=0.001;Group11、12and13,P=0.000).Conclusion:The frequency of lymph node metastasis significantly correlate with size of primary tumor,visceral pleura invaded and location of tumor.In terms of tumor size,except for the 7th group of lymph nodes,the metastasis rate of cT1a stage was lower than that of c T1 c stage.(Group2、3、4、5 and 6 P=0.003;Group10 P=0.017;Group8 and 9P=0.027;Group11、12and13 P=0.000).There was no statistical difference between stage cT1a and stage c T1 b,stage c T1 b and stage c T1 c,but there was no statistical difference between size and lymph node metastasis in the case of visceral pleural invasion.For tumor location,the lymph node metastasis rate of the upper left lobe was higher than that of the lower left lobe in Group 5 and 6(P=0.021).In Group 8 and 9 lymph nodes,the metastasis rate of the left upper lobe was lower than that of the left lower lobe(P=0.000).For Group 7,the metastasis rate of the lower lobe of the right lung was higher than that of the upper lobe of the right lung(P=0.000).Conclusions: Lymph node metastasis status of lung cancer below 3cm is affected by invasion status of visceral pleura,tumor size and tumor location.
