Comparison Of The Clinical Effect Of Uniportal Video-assisted Thoracoscopic Lobectomy And Biportal Video-assisted Thoracoscopic Lobectomy In The Treatment Of Lung Cancer

Objective: To investigate the feasibility of uniportal video-assisted thoracoscopic radical resection of lung cancer in the treatment of stage I to part of stage IIIA lung cancer.Methods: The clinical data of patients accepted thoracoscopic radical resection of lung cancer in a same medical group in the First Affiliated Hospital of Soochow University from May 2018 to Mar.2019 were collected and retrospectively analyzed.In total 142 cases were enrolled in this study after excluding cases which did not meet the requirements,and the patients were divided into uniportal group(55 cases)and biportal group(87 cases)respectively.Results: All the 142 lung cancer patients underwent lobectomy and systemic lymph node dissection.There is no perioperative death in both groups.There was no significant difference in age,location of tumors,pathological type,size of tumors(2.54±2.74 vs 2.56±1.82,centimeter),p TNM staging and number of lymph node dissections(15.73±7.87vs16.17±7.62,number)between uniportal group and biportal group(P>0.05).The operation time(167.65±43.85 vs 181.71±51.28,minutes),the intraoperative bleeding volume(57.45±50.19 vs 87.47±132.54,milliliter),the indwelling time of drainage tube(4.82±2.82 vs 5.84±3.43,days)and the hospital stay(6.91±3.88 vs 7.74±3.87,days)were less in uiportal group compared to biportal group,though no significant difference occurred(P> 0.05).The total drainage volume in uniportal group was significantly less than biportal group(1064.82±776.38 vs 1658.71±1722.38,milliliter),and the visual analogue score of 24 hours and 72 hours after operation(4.73±0.73vs5.25±0.74,2.16±0.71vs2.55±0.86)was significantly lower in uniportal group(P < 0.05).Conclusion: Uniportal video-assisted thoracoscopic radical resection of lung cancer is safe and feasible in stage I to parts of stage IIIA lung cancer patients,and it does not reduce the quality of operation compared with biportal video-assisted thoracoscopic surgery.It has the advantages of less pain,possibly faster recovery and fewer incisional scars after operation,and deserve the promotion.
