Objective: the effect of the relationship between the horizontal implant platform of maxillary central incisors and the position of homonymous teeth on the restoration aesthetics.Methods: from September 2017 to December 2019,the patients who were in the Department of Stomatology of The Fourth Affiliated Hospital of Inner Mongolia Medical University were selected.The upper and middle incisors were missing due to anterior teeth trauma,tooth disease,periodontal disease and other reasons,and the patients needed to carry out the ossem system delayed implant repair.The patients were divided into three groups according to the PES score of 6 months after repair: the first group was 42 patients with PES ≥ 12,the second group was 18 patients with PES ≥ 8,and the third group was 0 patients with PES < 8.The gap,width of homonymous teeth,implant radius,gap minus width difference of homonymous teeth,gap between implant and homonymous teeth,horizontal deviation,horizontal distance from the middle point of the maxillary central incisor implant platform to the middle line of homonymous teeth,vertical distance from the middle point of the upper central incisor implant platform to the plane of homonymous tooth cutting end and the distance from the slanted edge of the triangle were measured and calculated in 60 patients And the area of the triangle and Tan α(vertical distance divided by horizontal distance).We observed and measured the marginal bone resorption immediately after implantation and 6 months after restoration,PD,MBI and patient satisfaction survey immediately after restoration and 6 months after restoration to evaluate the influence of the relationship between the implant platform of maxillary central incisor and the tooth position of the same name on the restoration aesthetics.Since the third group included 0 patients,no statistical analysis was conducted.Results: 1.Statistical results of direct measurement data: there was no significant difference in the tooth gap(P = 0.387);there was significant difference in the width of teeth with the same name(P = 0.012);there was significant difference in the horizontal distance between the middle point of the upper central incisor implant platform and the middle line of the width of teeth with the same name(P = 0.032);there was significant difference in the vertical distance from the plane of the cutting end(P = 0.014).2.The statistical results of the two groups were calculated: there was no significant difference(P = 0.068)between the sum of the width of edentulous space and homonymous teeth divided by 2;there was no significant difference(P = 0.166)between the width of homonymous teeth reduced by the width of edentulous space;there was no significant difference(P = 0.319)between the horizontal deviation;there was no significant difference(P = 0.151)between the implant and homonymous teeth;there was no significant difference(P = 0.082)between the triangle bevel distance and the sum of the width of homonymous teeth There was no significant difference in triangle area(P = 0.425),and there was significant difference in Tan α value(P <0.001).3.Statistical results of marginal bone resorption(MBL)in the two groups: there was no significant difference between the proximal and middle marginal bone resorption(P = 0.133)and the distal and middle marginal bone resorption(P = 0.476).4.Statistical results of soft tissue in the two groups: there was a significant difference immediately after the repair of peri implant depth(PD)(P = 0.002);there was a significant difference 6 months after the repair of peri implant depth(PD)(P= There was a significant difference(P = 0.001)immediately after the repair of the modified sulcus bleeding index(MBI),and a significant difference(P = 0.044)6 months after the repair of the modified sulcus bleeding index(MBI).5.Statistical results of patients’ satisfaction survey: there was no significant difference in patients’ satisfaction with soft tissue color immediately after repair(P = 0.291);there was no significant difference in chewing function(P = 0.191);there was no significant difference in attachment height(P = 0.054);there was significant difference in overall aesthetics(P = 0.001).Six months after the restoration,there was a significant difference in patients’ satisfaction with soft tissue color(P = 0.011);there was no significant difference in masticatory function;there was no significant difference in attachment height(P = 0.134);there was a significant difference in overall aesthetics(P = 0.040).Conclusion: 1.The width of the central incisor can affect the aesthetic effect of the same name dental implant,and the distance can affect the aesthetic effect of the same name dental implant.2.It is suggested to adjust the size of homonymous teeth by means of restoration.After adjusting the space of orthodontics to affect the horizontal distance,implant restoration should be carried out.2.In the vertical direction,the vertical distance affects the implant depth and thus affects the aesthetic effect.It is suggested that the implant depth be determined after measurement.When designing the anterior teeth implant,the alveolar bone height should be preserved by the extraction site preservation or the bone should be removed or implanted.3.Tan α value has significant influence on the aesthetic effect of anterior teeth implant.It is suggested that the implant design before treatment should be coordinated with the ratio of vertical distance and horizontal distance,i.e.the aesthetic effect after the restoration of Tan α value in the range of 1.400 ± 0.208 is more ideal.
