| Objective:Tumor is the most common disease in salivary gland tissue.The incidence of parotid gland tumor is the highest in different anatomical parts of salivary gland,accounting for about 80%[1-3].More than 80%of parotitis tumors were located in the superficial lobe of parotid gland,showing masses under earlobe,preauricular area or posterior lower part of parotid gland.The treatment of parotid tumor is mainly surgery and the surgical resection should be carried out in the normal tissue outside the capsule,and partial or the whole gland should be resected at the same time.In recent years,the tumor and partial parotidecectomy has been widely used in the surgical treatment of parotid tumor.The histologic classification of parotid tumors is complex and different types of tumors are different in clinical manifestations,imaging manifestations,treatment and prevention.In this study,CT、pathological diagnosis results and tumor maximum diameter of 197 patients with parotid gland tumors were analyzed.The aim of study is to understand the relationship between parotid body surface division and the pathohistological type of the tumor,maximum tumor diameter,which will be helpful to the diagnosis,surgical treatment and prognosis of parotid tumors.Methods:197patients with parotid gland tumors treated surgically in the Department of Stomatology,No.1 Hospital of Qinhuangdao,from January2013 to December 2018 were collected and followed up for 1 year to 6 years.The parotid body surface zoning method provided by Xu Xiangliang,etc.was med to determine the zone of the surface of parotid gland.The marking points include nasal alar,oral angle,earlobe and posterior edge of mandibular branch then two lines of A,B were determined and the parotid gland was divided intoⅠ,Ⅱand III areas.The parotid gland CT of 197 patients was analyzed to determine the location of the tumor and the maximum tumor diameter.Using IBM SPSS Statistics 21.0 statistical analysis software,χ2 test and Fisher accurate test was carried out to analyze the division and histological type of parotid gland tumors.ANOVA and LSD method was carried out to analyze the division and maximum diameter of parotid gland tumors.The t test was used to analyze maximum tumor diameter growing across divisions and maximum tumor diameter not growing across divisions.Results:(1)Using theχ2 test and the Fisher accurate test on the distribution of different tumor histological types in different parotid divisions found that the incidence of Warthin tumor in area I was higher than other tumor histological types,and the incidence of pleomorphic adenoma in area I/III and area I/II/III was higher than ther tumor histological types(P<0.05).(2)ANOVA and LSD analysis of the maximum tumor diameter in different parotid divisions showed that the maximum tumor diameter in area I,area I/III and area I/II/III were larger than that in area II,and the difference was statistically significant(P<0.05).(3)The t test was used to analyze maximum tumor diameter growing across divisions and maximum tumor diameter not growing across divisions.it was found that the maximum tumor diameter growing across divisions was larger than the maximum tumor diameter not growing across divisions,and the difference was statistically significant(P=0.022).Conclusion:(1)Warthin tumor is more common in area I.Pleomorphic adenoma is more common in area I/III and area I/II/III.(2)Tumors growing in area II have a smaller average diameter than other areas.(3)The maximum tumor diameter growing across divisions was larger than the maximum tumor diameter not growing across divisions. |
