Objective: The purpose of this study was to evaluate the effectiveness and safety of drug-coated balloons in large coronary arteries.Methods: This experiment retrospectively analyzed patients with acute coronary syndrome who underwent percutaneous coronary intervention at the Second Hospital of Hebei Medical University from October 2016 to March 2019.There were 99 patients with 2.8 mm orthotopic lesions receiving DCB treatment,a total of 110 lesions.Through clinical follow-up and review of angiography.The minimum luminal diameter,advanced luminal loss,and major cardiovascular adverse events were evaluated before PCI,immediately after surgery,and follow-up on angiography.Results:1.In the baseline data of patients,the age was 55.13 ±11.81,81.8% for men,63.6%for smoking history,57%for hypertension,43.4%for hyperlipidemia,and 24.2% for diabetes.2.Baseline data of surgery.The length of the DCB was 22.13 ± 4.02 mm.The straight diameter of the DCB was 2.86 ± 0.33 mm.The inflation pressure of the DCB was 7.22 ± 0.42 atm.The inflation time of the DCB was 36.41 ± 12.04 s.Cutting and NSE balloon was 100%.The incidence of dissection after DCB dilation was 21 lesions,including 10 in type A and 8 in type B and 3 in type C.3.Coronary angiography was performed in 54(54.5%)of the 99 patients.The minimum lumen diameter before PCI was 0.59±0.21 mm,and the minimum lumen diameter immediately after surgery was 2.73±0.35 mm.There is a significant statistical difference between the two outcome.The average follow-up time of angiography was 4.52±2.79 months.The minimum lumen diameter during follow-up was 2.77±0.37 mm,which was significantly different from the minimum lumen diameter after surgery.Late official cavity loss was-0.04±0.16 mm.4.Ninety of the 99 patients in this experiment were followed up for 11.8 ± 8.5 months.Two cases of intravascular restenosis occurred during the follow-up(one of them did not undergo target lesion revascularization),and one patient had target lesion reconstruction.1 case of myocardial infarction(angiography showed non-target lesions caused by stenosis)and 3 cases of MACE events.No cardiovascular death occurred during follow-up,all due to death.Twenty of the 21 patients with dissection were followed up by angiography and healed completely.Conclusions:The drug-coated balloon is safe and effective in the large coronary arterial.
