Peroneal Flap: Ultrasound Study And Clinical Application

Objective: The distribution and hemodynamic parameters of peroneal perforator were studied by using color doppler ultrasonography.To investigate the anatomical structure of radial artery perforator and the clinical results of reconstruction with peroneal artery perforator flaps.Methods: 1.Color Doppler ultrasonography was used to study the number,distribution,type and length of brachial artery perforator vessels in 15 healthy volunteers.2.25 patients were treated with peroneal perforator flaps.Among them,17 patients had retrograde island radial artery flaps,4 patients had antegrade island radial artery flaps,and 4 patients had free radial artery flaps.The average age of the patients was 52.4 years old,and the size of the defect was 4.0×3.0cm to 15.0×10.0cm.Partial flapnecrosis occurred in 7 cases,4 of which required secondary surgery using skin grafting.No other sequelae occurred.Results: 1.A total of 15 healthy volunteers had 30 iliac arteries.The total number of perforators was 110 and the mean inner diameter at the beginning of perforation was 1.06±0.21 mm.2.There are 29 perforators in the proximal segment,53 perforators in the middle segment,and 28 perforators in the distal segment.Among them,45 branches(40.9%)were musculocutaneous perforators and 65(59.1%)were septocutaneous perforators.25 patients were treated with peroneal perforator flaps.Partial flapnecrosis occurred in 7 cases,4 of which required secondary surgery using skin grafting.No other sequelae occurred.Conclusions: The peroneal artery perforator flap is a good alternative for the reconstruction of soft tissue defects,with a constant and reliable vascular pedicle,thin and pliable skin,and the possibility of creating a composite tissue flap.
