Objective:To observe the effect of moxibustion on the content of β-endorphin(β-EP),Dynorphin(Dyn),Tumor necrosis factor-α(TNF-α),Interleukin-1β(1L-1β)in the serum of rheumatoid arthritis(RA)patients;to explore the mechanism of moxibustion to improve the anti-inflammatory and analgesia of RA.Methods:Using random control method,Sixty-four RA patients were randomly assigned to the control group and the treatment group.The control group: taking routine western medicine treatment,methotrexate(7.5mg/week),folate(10mg/week)or leflunomide(10mg/day).On the basis of routine western medicine,the treatment group was treated with moxibustion Zusanli,Shenshu,and Ashi points.Two times a week,one treatment every 3 days,four weeks as a course of treatment,continuous treatment for two course.Before and after treatment,the clinical symptoms,routine inspection indexs(ESR,CRP,RF),and the contents of β-EP,Dyn,TNF–α,1L-1β in the serum which were determined by ELISA of the two groups of RA subjects were compared.SPSS24.0 statistical software was used to analyze the two groups of data.Results:1.Baseline results: disease-related data,clinical symptoms(VAS score、morning stiffness score、DAS28 score),routine inspection indexs(RF、ESR、CRP)and levels of β-EP,Dyn,TNF-α,and 1L-1β in serum in the two groups before treatment consistent and comparable(P > 0.05).2.The effect of moxibustion on clinical symptoms(VAS score、morning stiffness score、DAS28 score)and routine inspection indexs(RF、ESR、CRP)of RA subjects:(1)Intra-group comparison: The clinical symptom indexes and l routine inspection indexs were improved before and after treatment in the two groups of RA subjects(P < 0.05).(2)comparison between the two groups: after the treatment of RA subjects in the two groups,the improvement of clinical symptoms in the experimental group was better than those in the control group(P <0.05),and there was no significant difference in the improvement of routine inspection indexs between the two groups(P>0.05).3.The effect of moxibustion on serum levels of β-EP,Dyn,TNF-α,1L-1β in RA subjects:(1)Intra-group comparison: before and after treatment the serum levels of β-EP,Dyn,TNF-α,1L-1β in the experimental group were all improved(P <0.05)and in the control group were not significantly improved(P > 0.05).The improvement of analgesic and inflammatory factors before and after treatment was more obvious in the experimental group.(2)Comparison between groups: After treatment of RA subjects in the two groups,the levels of β-EP,Dyn,TNF-α,and 1L-1β in the serum of the test group improved better than those in the control group(P <0.05).The improvement of analgesic and inflammatory factors was more obvious in the experimental group than in the control group.4.Correlation analysis of β-EP and Dyn with TNF-α and 1L-1β: The difference between β-EP and Dyn in the serum of the subjects before and after treatment in the experimental group was correlated with the difference between TNF-α and 1L-1β,and was mainly negatively correlated(r <0).5.Safety evaluation results: Gastrointestinal adverse reactions often occur in oral conventional western medicines.The gastrointestinal adverse reactions in the moxibustion moxibustion test group were few.Conclusion:1.Conventional western medicine can stabilize the condition of RA patients and reduce the clinical symptoms of patients.Moxibustion can enhance the relief of joint pain of conventional western medicine on RA patients and improve joint mobility and swelling.2.The improvement of clinical symptoms and pain relief in the experimental group is significantly better than that in the control group.It may enhance the effect of conventional western medicine on serum β-EP and Dyn levels in RA patients,as well as inflammatory factors TNF-α and 1L-1β content is related.3.Moxibustion enhances the improvement of the pain of conventional western medicine on RA patients.but its anti-inflammatory analgesic mechanism needs more comprehensive and in-depth discussion.One of its anti-inflammatory and analgesic effects may be to play an anti-inflammatory effect by affecting inflammatory factors,and by affecting β-EP and Dyn play an analgesic role.But its mechanism needs more comprehensive and in-depth discussion.4.Moxibustion is safe and effective in the treatment of RA,and may reduce the side effects of conventional western medicine.
