Objective:To observe the local efficacy of particle implantation in stage Ⅳ non-small cell lung cancer(NSCLC),and to explore the significance of particle implantation as a palliative treatment in stage Ⅳ non-small cell lung cancer.Methods:We collected stage Ⅳ NSCLC patients admitted to our hospital for 125 I seed implantation from August 2015 to September 2019,select appropriate patients according to inclusion and exclusion criteria,and record the basic information and clinical data of patients.The basic information includes: sex,age,Smoking Status,pathological type,focus size and operation site.Laboratory and imaging examination: preoperative hemoglobin,CT of the implantation site before and 3 months after operation.Clinical symptoms: cough,chest distress and pain before and 3 months after operation,postoperative adverse reactions recorded.Follow-up information: the local control rate at 3 months after operation was recorded and the influencing factors of the local control rate were an alyzed.The remission of clinical symptoms before and 3 months after o peration was compared to evaluate the efficacy of particle implantation in relieving clinical symptoms and analyze the factors related to local control rate and clinical symptom relief.Results:1 Imaging effect: the local control rate was 76.4% 3 months after operation;tumor size and hemoglobin level were independent risk factors for local control rate.When the tumor diameter≤3cm and hemoglobin≥120g/L,the local control rate increased significantly.2 Relief of clinical symptom:The cough relief rate was 62.7%(32/51),chest tightness relief rate was 53.5%(23/43)and pain relief rate was 72.9%(43/59)in 3 months after operation.Compared with preoperative score,the difference was statistically significant(P<0.05).3 The relationship between imaging effect and clinical symptom relief:The correlation analysis of imaging effect and clinical symptom relief was made,p<0.05,there was a positive correlation between imaging effect and clinical symptom relief.4 Adverse reaction : The adverse reactions related to particle implantation: pneumothorax 43.4%(23/53),bleeding 6.7%(5/89),particle displacement 1.4%(1/89).After symptomatic treatment,the above symptoms were improved,and no serious adverse reactions were observed during the follow-up period.Conclusion:1 Because of its high local control rate,radioactive particle implantation can significantly relieve clinical symptoms and effectively improve the quality of life of patients with advanced non-small cell lung cancer.2 Radioactive particle implantation is a safe and effective local treatment method with less adverse reactions and short operation time.It provides a new treatment opportunity for patients with advanced stage non-small cell lung cancer,especially the Elderly and infirm patients in general poor condition.
