With the continuous improvement of living conditions,people have hi gher requirements for the quality of daily life.At the same time,more and more attention has been paid to the pain caused by rotator cuff tears.But still exist most people to rotator cuff tears understanding is not enough,mistake for shoulder periarthritis and delay even missed the best treatment opportunity;Because rotator cuff tear is very common in life,and seriously affects the daily life of patients,and reduces the quality of life,therefore,the exploration of a convenient,cheap and efficient diagnosis method in clinical practice will bring good news to patients.Objective:The imaging findings of musculoskeletal ultrasound and MRI were compared and compared with the diagnosis results of arthroscopy.To evaluate the diagnostic value of musculoskeletal ultrasound and MRI in rotator cuff tears.Methods:Collect the history,clinical presentation,and physical examination and suspected patients with rotator cuff tears admitted arthroscopic shoulder surgery treatment of 56 patients,56 patients with preoperative conventional line shoulder joint(double side)musculoskeletal ultrasound and MRI,and check with arthroscopic shoulder as the gold standard in the diagnosis of rotator cuff tears,using measurement data matching chi-square test(Mc Nemar test)analysis of two kinds of clinical imaging examination method of rotator cuff tears,the difference of the evaluation of musculoskeletal ultrasound and MRI for detection the sensitivity of the rotator cuff tears,specific degree,accuracy,positive predictive value,negative predictive value.The degree of rotator cuff tear was evaluated by musculoskeletal ultrasound and MRI.Results:Among the 56 patients who underwent shoulder arthroscopy,the total rotator cuff tear in shoulder arthroscopy accounted for 48.21% in 27 cases,MRI accounted for 42.86% in 24 cases,and musculoskeletal ultrasound accounted for 46.43% in 26 cases.Among the partial tears,shoulder arthroscopy accounted for 28.57% in 16 cases,MRI accounted for 30.36% in 17 cases,and musculoskeletal ultrasound accounted for 25.00% in 14 cases.There was no significant difference in the detection rate of full-thickn ess tear of rotator cuff by musculoskeletal ultrasound and arthroscopy(x~2=0.036,P=0.850).There was no significant difference between MRI and arthr oscopy in the detection rate of rotator cuff tear(x~2=0.324,P=0.569).The di fference in the detection rate of partial rotator cuff tear by musculoskeletal ultrasound and arthroscopy was not statistically significant(x~2=0.182,P=0.670).There was no significant difference in the detection rate of partial rot ator cuff tear detected by MRI and arthroscopy(x~2=0.043,P=0.836).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of rotator cuff tears were evaluated by arthroscop y.Complete tear: ultrasonography: 85.19%,89.66%,87.50%,88.46%,86.67%;MRI results were: 85.19%,96.55%,91.07%,95.83%,87.50%,respectiv ely.Partial tearing: ultrasound: 75.00%,95.00%,89.29%,85.71%,90.48%;MRI results were: 87.50%、92.50%、91.07%、82.35%、94.87%,respectiv ely.Area under ROC curve: musculoskeletal ultrasound and MRI were 0.981 and 0.944 for complete tear,respectively.Partial tear 0.938,0.988.Conclusions:Musculoskeletal ultrasound and MRI showed high sensitivity,specificity and accuracy in rotator cuff tear.Due to the expensive price,complicated operation,long time and contraindications of MRI,some people cannot get a diagnosis.However,musculoskeletal ultrasound has the advantages of low price,repeatable operation,dynamic observation,and accurate intraoperative positioning of the injured site,so it can be used as the first choice for rotator cuff tear screening and postoperative follow-up,and widely used in the diagnosis of shoulder joint diseases.
