| Objective To explore the clinicopathological factors that influence the concurrent peritoneal metastasis of colorectal cancer. Methods The clinical data of 6505 patients with colorectal cancer in the Affiliated Hospital of Qingdao University from 2009 to 2019 were collected retrospectively, including age(age,y),tumor markers including carcinoembryonic antigen(CEA,ng / ml),carbohydrate antigen 19-9(CA19-9,U / ml),hemoglobin(Hb,g / L),albumin(ALB,g /L),patient gender, previous abdominal operation history, diabetes history, abdominal symptoms(abdominal pain, abdominal distention, etc.),primary intestinal tumor features include tumor location, tumor differentiation degree, tumor pathological type, other simultaneous features of intestinal tumor include lymph node metastasis, liver metastasis, ascites,a total of 15 indicators.Patients were divided into peritoneal metastasis group and non peritoneal metastasis group according to whether simultaneous peritoneal metastasis occurred.Among them,t-test was used for measurement data, rank sum test and chi square test were used for single factor analysis for counting data, and the relevant risk factors were screened out,and then unconditional logistic regression was used for multi factor analysis. Results Among 6505 patients with colorectal cancer,225(3.46%)had peritoneal implant metastasis.Single factor analysis showed that carcinoembryonic antigen, carbohydrate antigen 19-9,albumin,hemoglobin,patient’s gender, abdominal symptoms, tumor location, tumor pathological type, tumor differentiation degree, other simultaneous characteristics of intestinal tumor including lymph node metastasis, liver metastasis and ascites all showed that they would have a Pnificant impact on simultaneous peritoneal metastasis of colorectal cancer.Unconditional logistic multivariate analysis showed that abdominal symptoms, tumor markers carcinoembryonic antigen, carbohydrate antigen19-9,albumin,tumor location, tumor differentiation degree, other simultaneous characteristics of intestinal tumor including lymph node metastasis, liver metastasis, ascites and simultaneous peritoneal metastasis of colorectal cancer(P < 0.05).Conclusion Abdominal symptoms, tumor markers carcinoembryonic antigen, carbohydrate antigen 19-9,albumin,tumor location, tumor differentiation, lymph node metastasis, liver metastasis and ascites are all risk factors for concurrent peritoneal metastasis of colorectal cancer. |
