| Objective: To analyze the distribution of pathogenic bacteria and antibiotic resistance in patients with LRTI in the intensive care unit of our hospital,which may help to select antimicrobial drugs more rationally and provide more ideas and theoretical basis for the clinical diagnosis,treatment and management of LRTI resistance.Methods: Through our hospital’s inspection LIST system,we collected the pathogenic bacteria isolates that were positive for bacterial culture in our hospital’s laboratory and surgical intensive care unit from January 2016 to December 2017,and then screened out through our hospital’s electronic medical records and imaging system software.A total of 545 strains that met the inclusion criteria were analyzed retrospectively.Results: A total of 545 strains that met the inclusion criteria were screened out,including 449 Gram-negative bacilli and 96 Gram-positive cocci.AB was the main gram-negative bacilli,accounting for 32.11%,followed by PA,accounting for 18.35%.Among Gram-positive cocci,CNS ranks first,followed by SA.In AB,CRAB is the main species,and its sensitivity to tigecycline and amikacin can reach more than 80%.The sensitivity of PA to piperacillin/tazobactam,gentamicin,imipenem,meropenem,ciprofloxacin,levofloxacin is between 50% and 80%,and to amikacin and cefoperazone The sensitivity rates of ketone/sulbactam and tobramycin were all 80%and above.Extended-spectrum β-lactamase-producing KP and Eco susceptibility results showed that for carbapenems,β-lactam/β-lactamase inhibitors,tigecycline,quinolones,amikacin were greater than 60%.Among Gram-positive cocci,MRCNS and MRSA were 100% sensitive to nitrofurantoin,quinupristin/dafopritin,tigecycline,and vancomycin,the sensitivity rates to rifampicin and linezolid were greater than70%.Conclusion: Gram-negative bacilli are the most common among LRTI pathogenic bacteria in the intensive care unit,of which Acinetobacter baumannii and Pseudomonas aeruginosa are the main species.Both resistant and non-resistant strains are sensitive to amikacin and Tigecycline.Staphylococci account for about one-half of the total number of Gram-positive cocci.Among them,methicillin-resistant strains have good sensitivity to vancomycin and linezolid and can be included in empirical treatment. |
