| Object:The aim was to observe the clinical efficacy of Sun’s abdominal acupuncture catgut embedding in the treatment of chronic insomnia and its effect on plasma neuropeptide Y(NPY)levels,to explore the possible mechanism of this therapy in treating chronic insomnia,and to provide some scientific basis for the treatment of chronic insomnia by Sun’s abdominal acupuncture catgut embedding.Method:Sixty-six patients with chronic insomnia who met the inclusion criteria were randomly divided into a treatment group(abdominal acupuncture catgut embedding group)and a control group(conventional acupoint embedding group)with 33 cases in each group.The acupoints in the treatment group were abdominal one,abdominal two,and abdominal eight.The acupoints of the control group were Zhaohai(two sides),Shenmai(two sides),Shenmen(two sides),Sanyinjiao(two sides),Anmian(two sides)and Sishencong.Both groups used the minimally invasive thread embedding method,once every 10 days,once for a course of treatment,for a total of 2 courses of treatment.Changes in plasma NPY levels,Pittsburgh sleep quality index(PSQI),self-rating anxiety scale(SAS),and self-rating depression scale(SDS)were compared between the two groups before and after treatment.Data analysis was performed using SPSS26.0 software.Result:1.There were 4 patients who had fallen off partway through the trial.The actual number of completed trials was 62,32 in the treatment group and 30 in the control group.Before treatment,there was no significant difference in the general conditions(gender,age,duration),plasma NPY level,PSQI score,SAS score,and SDS score between the two groups(P>0.05),which were comparable.2.Clinical efficacy:After 2 courses of treatment,the total effective rateof the treatment group was 93.75%,which was better than the control group’s80.00%,with a significant statistical difference(P<0.01).3.PSQI score: After treatment,the PSQI total score and the score of each component were significantly improved compared with those before treatment,and the difference was statistically significant(P<0.05);Comparing the total PSQI scores and components after treatment between the two groups,the treatment group was better than the control group in terms of total score,sleep quality,and daytime dysfunction,with significant statistical differences(P<0.01);In terms of sleep disorders,there was a statistical difference(P<0.05);and there was no statistical significance in terms of sleep time,sleep time,and sleep efficiency(P>0.05).4.NPY level: The levels of NPY in the two groups increased after treatment compared with those before treatment,with a significant statistical difference(P<0.01);After treatment,the NPY content of the treatment group was significantly higher than that of the control group,and there was a statistically significant difference(P<0.01).5.SAS score: The SAS scores of the two groups decreased after treatment compared with those before treatment,with a significant difference(P<0.01);There was a significant difference between the groups after treatment(P<0.01).6.SDS score: The SDS scores of the two groups decreased after treatment compared with those before treatment,with a significant difference(P<0.01);There was a significant difference between the groups after treatment(P<0.01).7.Follow-up: One month after the end of treatment,the follow-up results showed that the relapse rate of the treatment group was 13.33%,and the relapse rate of the control group was 41.67%.The difference between the two groups was statistically significant(P<0.05).Conclusion:1.Sun’s abdominal acupuncture catgut embedding and conventionalacupoint embedding can effectively treat chronic insomnia,both can improve patients’ sleep,anxiety and depression.2.The clinical efficacy of Sun’s abdominal needle embedding in the treatment of chronic insomnia is superior to that of conventional acupoint embedding,and it has more advantages in sleep quality,sleep disorders,daytime dysfunction and long-term efficacy.3.The possible mechanism of Sun’s abdominal acupuncture catgut embedding treatment for chronic insomnia is to directly stimulate the neurons of the enteric nervous system,correspondingly regulate the emotional area,autonomic area and endocrine regulation area of the brain,increase the secretion of neuropeptide Y,and thus improve Sleep. |
