Objective:The expression of serum adiponectin,tumor marker and miR-10b in the serum of patients with gastric cancer was detected.The diagnostic basis of serum adiponectin as tumor marker of gastric cancer was studied.The clinical value and clinical application prospect of the above indicators in the diagnosis of gastric cancer were evaluated.Methods:There were 156 cases in the study,96 of them were in the experimental group and 60 of them were in the control group.The expression of adiponectin in serum was detected by ELISA,CEA,CA19-9,CA125 and CA724 by chemiluminescence and miR-10b by qRT-PCR.The statistical analysis was carried out by spss24.0.X test was used to compare the counting data between the two groups.The measurement data of normal distribution is expressed by X ± s,and the comparison is made by t test;the measurement data of non normal distribution is expressed by median,the comparison is made by nonparametric rank sum test,and the comparison between the two groups is made by Mann Whitney U test.Spearman correlation test was used for correlation analysis.P<0.05 was statistically significant.ROC curve analysis was used to evaluate the value of screening serum adiponectin,CEA,CA199,CA125,CA724,CA242 and miR-10b in the diagnosis of gastric cancerResults:The median level of adiponectin in gastric cancer group was 5.42 μg/ml,which was significantly lower than that in healthy control group(14.23 μg/ml).The difference between the two groups was statistically significant(P<0.05).The gastric cancer group was further divided into early gastric cancer group and advanced gastric cancer group.The median level of adiponectin was 8.15 μg/ml in the early gastric cancer group and 4.56 μg/ml in the advanced gastric cancer group.The difference between the two groups was statistically significant(P<0.05).The results of adiponectin were IA 7.63(4.64,11.61),IB8.15(4.18,9.94),ⅡA 4.08(1.71,10.09),ⅡB 7.32(1.92,35.96),IIIA 4.20(2.55,6.20),ⅢB 4.60(2.63,8.94),Ⅲc 3.54(2.14,9.25),IV5.41(5.41),5.41);there was no statistical difference between different stages(P>0.05),and then further divided phase Ⅰ and phase Ⅱ into a group,phase Ⅲand phase Ⅳ into a group.The results of phase Ⅲ and phase Ⅳ group were 4.41(2.57,6.53),significantly lower than that of phase Ⅰ and phase Ⅱ group by 7.06(4.02,11.01),P<0.05.The area under ROC curve was 0.828,95%CI(0.817,0.951),sensitivity was 65.5%,specificity was 94.9%,and the area under ROC curve was 0.884,95%CI(0.817,0.951),sensitivity was 85.1%,specificity was 89.8%.The combined detection of the two indexes has diagnostic significance for early gastric cancer and advanced gastric cancer.The area under ROC curve of adiponectin combined with tumor markers is the largest,which is more significant than the application of tumor markers alone.The sensitivity and specificity of 0.908,95%CI(0.845,0.971)were 88.1%and 89.8%respectively.The other order is CEA(0.806)CA199(0.738)>CA724(0.719)>CA125(0.708)>CA242(0.645).conclusion1.The content of serum adiponectin in patients with gastric cancer decreased,and the decrease of advanced gastric cancer was more obvious than that of advanced gastric cancer.With the aggravation of pathological stage,adiponectin decreased more significantly.2.Adiponectin and miR-10b can be used in the diagnosis of gastric cancer.3.The combined diagnosis of adiponectin and tumor markers is more significant than single tumor marker.
