Objective:To compare the clinical effects of ganciclovir combined with dexamethasone and dexamethasone alone in the treatment of acute idiopathic facial neuritis.Methods:From December 2015 to February 2017,80 patients who met the diagnostic criteria of acute idiopathic severe facial neuritis were randomly divided into treatment group and control group according to the order of admission,40 in each group.Treatment group:mecobalamin injection 1mg / time,intramuscular injection,once a day;vitamin B1 10 mg oral,three times a day,combined with ganciclovir injection 5mg / kg,intravenous drip,twice a day,course of treatment for 10 days;dexamethasone sodium phosphate injection10 mg / time,once a day,five days after continuous use,changed to oral and gradually reduced to stop.Control group: mecobalamin injection 1mg / time,intramuscular injection,once a day;vitamin B1 10 mg oral,three times a day,dexamethasone sodium phosphate injection,10 mg / time,once a day,changed to oral after 5 days of continuous use and gradually reduced to stop.All patients were followed up for(9.2 ± 3.2)months to record the general situation and adverse reactions in the course of treatment.All patients were compared before treatment,1 week after treatment and 3 months after treatment.At the end of the last follow-up,the scores of the rating system of facial nerve function(hbgs)and the simple rating scale of portmann were recorded before and 2weeks after treatment The amplitude of motor evoked potential(AMP),conduction velocity(NCV)and electromyogram(EMG)of facial nerve were measured by SPSS18.0.Results:There was no significant difference between the two groups in general information,age,gender,course time,position of facial paralysis and degree of facial paralysis(P > 0.05).1.Comparison within the group: the difference between the treatment group and the pre-treatment group was statistically significant(t = 1.432,P < 0.05);the treatment group and the one week treatment group were statistically significant(t = 1.582,P < 0.05);the control group and the control group were statistically significant(11.22 ± 2.12)andThere was a significant difference in the score(2.70 ± 2.30)before treatment(t = 0.923,P < 0.05).There was no significant difference in the score(12.98 ± 1.50)after 3 months of treatment in the control group compared with the score(11.22 ± 2.12)after 1 week of treatment(t = 1.492,P > 0.05).2.Comparison between groups: there was no significant difference between the two groups before treatment(P > 0.05);there was no significant difference between the two groups after one week of treatment(12.11 ± 1.62)and the control group(11.22 ± 2.12)(t = 1.633,P > 0.05);there was significant difference between the two groups after three months of treatment(P < 0.05).Statistical results of electromyography: there was statistical significance between the two groups before and after treatment(P < 0.05).There was no statistical significance between the two groups after treatment(P > 0.05).There was no statistical significance between the two groups(P > 0.05)The wave amplitude was higher than that of the control group,indicating that the prognosis of the treatment group was better than that of the treatment group.Conclusion:The results of this study show that ganciclovir combined with dexamethasone and dexamethasone alone have a significant effect on the treatment of acute idiopathic severe facial neuritis,and the effect of steroid hormones in the early stage is obvious,but the long-term effect is not as good as that of antiviral drugs in the early stage The effect of ganciclovir combined with dexamethasone in the treatment of acute idiopathic severe facial neuritis is significant,and the prognosis is better than steroid alone.
