The Diagnosis And Surgical Treatment Of Appendiceal Mucinous Tumor

Objective: To explore the diagnosis and surgical treatment of the appendiceal mucinous tumor.Methods: The clinical data of 24 patients with the appendiceal tumor,treated in the Fourth Affiliated Hospital of China Medical University Since January 2007 to December2017 were retrospectively analyzed.The data of 24 patients meeting the criteria,including age,gender,clinical manifestations on admission,imaging examination,serum tumor marker level,surgical method,postoperative pathological results,chemotherapy regimen,prognosis and recurrence,were analyzed.Results: Among the 24 patients,There were 8 males and 16 females.The age ranges from 46 to 83,The median age was 62.Preoperative CT examination showed 6 cases of acute appendicitis,6 cases of ileocecal masses,2 cases of appendiceal myxadenoma,8cases of appendix mucocele,and 2 cases of intestinal obstruction.Appendicitis and abdominal mass were the major clinical manifestations of appendiceal mucinous tumor.All 24 patients received surgical operation and confirmed by the histopathological diagnosis.There were 2 cases for mucinous cystadenoma of the appendix,12 cases for low-grade appendiceal mucinous tumor,10 cases for appendiceal mucinous cystadenocarcinoma.All of them were treated with surgical resection,including single appendectomy in 5 cases,right-side colonectomy in 15 cases,ileocecoectomy in 4 cases.All 24 patients were postoperative followed-up until December 1,2018.The 2 cases died in second and third years after operation,3 cases with adenocarcinoma died of other diseases,And 1 case with adenocarcinoma was operated,bacause of tumor recurrence at one year after operation.No recurrence or metastasis has been found in the follow-up the patients remained.Conclusion: Appendiceal mucinous tumor which lack specific clinical features are uncommon and difficult to be diagnosed.In this study,through analyzing the clinical data of 24 confirmed patients and combining with the domestic and foreign literature,wefound that CT has a high clinical value in the diagnosis of appendiceal mucinous tumor,and can be used as the first auxiliary examination for the disease.Operation is the main treatment mean of appendiceal mucinous tumors,and operation choice is particularly important.The main surgeical methods were appendectomy,right-side colonectomy and ileocecoectomy.
