Objective:To explore the effects of water exercise therapy on the mean surface electro myography(AEMG),balance ability and gross motor function of the tibials anterior muscle,medial gastrocnemius head muscle,rectus femoris,semitendinosus semimem brane muscle of the lower extremity in children with cerebral palsy spasm from 3 to 6years old.Methods:1.Case selection: 30 children with spastic cerebral palsy aged 3-6 years from Q i Lu children’s hospital of Shan Dong university were randomly divided into spa group and control group,15 of whom were randomly divided into spa group and control gro up.A total of 23 children completed 12-week rehabilitation treatment,including 11 in the spa group(n=11)and 12 in the control group(n=12).2.Treatment methods: the control group was given 12-week exercise therapy,n erve electrical stimulation therapy and paraffin therapy;The spa group was given 12 weeks of water exercise therapy on top of regular training.3.Efficacy evaluation: before and after treatment,the average surface electrom yography(AEMG),Berg balance scale(BBS),gross motor function scale(GMFM)a nd area D and E of the two groups of children were evaluated.Results:1.Baseline comparison: before treatment,there were no significant differences in the age,gender,GMFCS grade,mean surface electromyography(AEMG),gross moto r function scale(GMFM)and area D and E score and BBS score between the two gro ups(P>0.05).2.Methods of electricity average electrical values(AEMG): spa group with strong side leg tibialis anterior muscle and weak side leg AEMG mean have increased after intervention and have significant difference before and after intervention(p <0.05),the control group with strong side leg tibialis anterior muscle and weak side leg AEMG average after intervention has increased but has no significant difference were observed in the before and after intervention(p > 0.05);Strong side leg medial head of the gastrocnemius muscle AEMG spa group after intervention average has decline and the difference before and after the intervention has significant(p < 0.05),the spa group of children with weak side and the control group with strong side,the weak side leg medial head of the gastrocnemius muscle AEMG mean have a downward trend after intervention but has no significant difference were observed in the before and after intervention(p > 0.05);The mean value of the muscle AEMG of the thigh rectus femoris muscle on the strong side and the weak side of the children in the spa group showed an increasing trend after intervention,and the difference was significant before and after intervention(p<0.05).The mean value of the thigh rectus femoris muscle on the strong side and the weak side of the children in the control group showed an increasing trend after intervention,but the difference was not significant before and after intervention(p >0.05).In the hydrotherapy group,the mean value of AEMG of hamstring semitendinosus semitememus on the strong side and the weak side showed a downward trend after intervention,and the difference before and after intervention was significant(p<0.05).In the control group,the mean value of the strong hamstring semitendinosemembrane muscle showed a downward trend after intervention and the difference before and after intervention was significant(P<0.05).The mean value of the weak hamstring semitendinosembrane muscle showed a downward trend after intervention,but the difference before and after intervention was not significant(P>0.05).3.Balance function: after treatment,the mean score of Berg balance scale was increased in the hydrotherapy group,and the difference before and after treatment was significant(p<0.05),while the mean score of Berg balance scale was increased in the control group,but the difference before and after treatment was not significant(p>0.05).4.Gross motor function: the mean score of zone D of gross motor ability of children in the spa group and the control group increased after treatment,and the difference before and after treatment was significant(p<0.05).After treatment,the mean scores of children in the hydrotherapy group showed a trend of increasing and the difference before and after treatment was significant(p<0.05).After treatment,the mean scores of children in the control group showed a trend of increasing but the difference before and after treatment was not significant(p>0.05).5.Comparison of differences between groups after treatment: there was no significant difference in various indexes between the spa group and the control group after treatment(p>,0.05).Conclusion:1.Water exercise therapy combined with conventional treatment has a good effect on improving the muscle strength of the anterior tibial muscle and rectus femoris,and reducing the muscle tension of the medial head and semitendinosus of gastrectus in children with spastic cerebral palsy.2.The combination of water exercise therapy and conventional treatment has a good effect on improving the balance ability and gross motor function(D and E)of children with spastic cerebral palsy.3.Conventional treatment has a good effect on reducing the tension of semimemius muscle of semitendinosus and improving the capacity of gross motor function(Area D).
