Objective:Under some pathological conditions,the erythrocyte sedimentation rate will be extremely increased.To analyze clinical diagnosis and laboratory examination of patients with extremely elevated ESR,to find out the factors that caused the extreme increase of ESR,and provide assistance for clinical diagnosis and treatment.Methods:A retrospective study was performed to analyze the clinical data of 429 patients with ESR≥100 mm/h in the First Affiliated Hospital of Kunming Medical University from from January 1,2019 to December 31,2019.Age,sex,clinical diagnosis,The value of the first ESR≥100mm/h and blood routine within 24 hours of admission(white blood cells,red blood cells,hemoglobin,hematocrit,platelets),liver function(albumin,globulin,alanine aminotransferase,aspartate aminotransferase,total bilirubin,direct bilirubin),renal function(urea,creatinine,uric acid),coagulation function(fibrinogen),C-reactive protein(CRP)were collected.The patients were divided into infectious disease group,hematological disease group,autoimmune disease group,renal failure group and other group according to clinical diagnosis.Analyze the distribution of extreme ESR in diseases and the correlation between extreme ESR and various various laboratory inspection indicators.Results:1.Among 429 patients,the average age was(53.70±18.70)years old,there were 236 males and 193 females,average ESR value(110.00±2.00mm/h).There was no significant difference in gender composition value between patients with ESR value,male(110.00±18.75mm/h)vs.female(111.00±36.00mm/h)(P=0.234).Compared with youth group,ESR value in middle-aged group and elderly group were decreased.2.The main diagnoses associated with extreme ESR[n(%)]were infection[157(37%)],blood disease[127(30%)],autoimmune disease[74(17%)].There were significant differences between pulmonary infection,hematopoietic stem cell disease,lymphocyte and plasma cell disease,red blood cell disease,diffuse connective tissue disease and ESR extreme rise.3.The extreme increase of ESR was correlated with levels of RBC,HB,HCT and FIB.The extreme increase of ESR was negatively correlated with levels of RBC,HB,and HCT,and positively correlated with level of FIB;there was no correlation between extreme increase of ESR and level of CRP.Conclusions:1.Extremely high ESR has nothing to do with gender.Compared with the youth group,the ESR value of the middle-aged group and the elderly group is higher.2.The main diagnoses associated with extreme ESR(n[%])were pulmonary infection,hematopoietic stem cell disease,lymphocyte and plasma cell disease,red blood cell disease,3.The extreme increase of ESR was negatively correlated with levels of RBC,HB,and HCT,and positively correlated with level of FIB;there was no correlation between extreme increase of ESR and level of CRP.
