Chronic Kidney Disease(CKD)has the characteristics of high incidence,low cognitive rate,poor prognosis and high medical costs.The health management time span of CKD is large,and it is often accompanied by chronic diseases such as diabetes,hypertension,and cardiovascular disease.To control disease progression and prevent complications,patients need long-term health management and intervention.Mobile health breaks the time and space limitations of traditional medical models and provides a new way of long-term management for patients outside the hospital.This thesis designs and implements a personalized management system for chronic kidney disease.The system is based on mobile health and developed in a cross-platform way,divided into doctor-side and patient-side.Combined with domestic and foreign CKD clinical practice guidelines,the system proposes CKD health management solutions,personalizes,standardizes,and systematicizes patient self-health management processes,which helps patients improve compliance and promote continuous health management.Its specific content is as follows:1、Formulate and generate personalized health management programs based on the characteristics of patients in different CKD stages,to meet the different management needs of patients,and reduce over-intervention and under-intervention.2、At the same time,it provides the visualization of patients’ long-term vital signs dada and decision-assistant module.Patients and doctors can understand the patient’s health status according to the visualization status and condition report,then judge the disease progress more accurately,and adjust the health management plan or intervene in time.3、In addition,referring to the CKD guidelines,the system also build the ontology of the chronic kidney disease field,and implement a doctor-patient communication assistant question-and-answer tool combined with natural language processing technology,,which helps doctors to handle patient consultation conveniently and quickly and promote doctor-patient communication.The system has been applied in out-of-hospital management for CKD patients.At present,there are more than 5,000 registered users,including about 2,400 medical staff and more than 3,000 patients,mostly in the stage of CKD 1-2. |
