ObjectivesIt was to investigate the sleep quality and influencing factors in colorectal cancer with enterostomy patients during chemotherapy,to understand the status quo of mindfulness level and coping style, which analyzed the relationship among them. The mediating effect of coping style between mindfulness level and sleep quality in colorectal cancer with enterostomy patients during chemotherapy was explored, which provided the basis for clinical medical staff to make intervention strategies in patients, and to improve the patients’ sleep quality.MethodsThis study used the convenience sampling method. From December 2018 to August 2019,a total of 360 colorectal cancer with enterostomy patients during chemotherapy who met the inclusion and exclusion criteria were selected to investigate from a tertiary Class A tumor hospital in Henan Province. At last, there were 352 valid questionnaires. The surveying tools included the general information questionnaire,Pittsburgh Sleep Quality Index(PSQI),Mindful Attention Awareness Scale(MAAS)and Medical Coping Modes Questionnaire(MCMQ).SPSS22.0 was used to process the data and statistical analysis. The general information of patients were expressed by the frequency and percentage. The total scores of each scale and dimension were expressed by the mean and standard deviation. The independent sample t test and analysis of variance were utilized for the comparison among groups.Pearson correlation analysis was used to analyze the relationship among the mindfulness level, coping style and sleep quality for patients during the processing of chemotherapy. The influencing factors of sleep quality were analyzed by multiple linear regression analysis during chemotherapy. The mediating affection of coping style between the mindfulness level and sleep quality was analyzed by means of sequential examination.Results1.The current status of sleep quality in colorectal cancer with enterostomy patients during chemotherapyThe total score of sleep quality in colorectal cancer with enterostomy patients during chemotherapy was(9.47±4.13),which was at the poor level. The scores of all dimensions from high to low were: daytime dysfunction(1.86±1.17),sleeping disturbance(1.67±0.54),falling asleep(1.64±0.91),sleeping efficiency(1.56±1.02),subjective sleeping quality(1.48±0.72),sleeping time(1.15±1.03),and hypnotic drugs(0.10±0.30).2.The current status of mindfulness level and coping style in colorectal cancer with enterostomy patients during chemotherapyDuring chemotherapy, the total score of mindfulness level in colorectal cancer with enterostomy patients was(50.14±9.61),which reached at the medium level. The total score of coping style was(44.38±5.38).It included three dimensions of facing, avoiding and yielding. The scores of all dimensions were(16.98±5.20),(16.22±3.86),(11.18±2.97).3.The univariate analysis of sleep quality in colorectal cancer with enterostomy patients during chemotherapyThey were carried by the univariate analysis to take the sociodemographic data and the related disease data as the independent variables, and take sleep quality during chemotherapy for patients as a dependent variable. The results showed that there were the statistically significant difference in scores of sleep quality at the different ages, the educational levels, per capita monthly income of families, enterostomy postoperative time,self-care status of enterostomy, enterostomy complications occurring or not, types of enterostomy, and the frequency of chemotherapy(p < 0.05).4.The correlation among the mindfulness level, coping style and sleep quality in colorectal cancer with enterostomy patients during chemotherapyThe results of the Pearson correlation analysis showed that the level of mindfulness was the negatively correlated with the total score of sleep quality in colorectal cancer with enterostomy patients during chemotherapy(r=-0.682,p < 0.01).The coping style of face-to-face was negatively correlated with the total score of sleep quality(r=-0.394,p < 0.01).The avoidance and yield coping styles were positively correlated with the total score of sleep quality(r=0.666,p < 0.01;r=0.568,p < 0.01).There was a positive correlation between the mindfulness level and the coping style of face-to-face(r=0.533,p < 0.01).There was a negative correlation among the mindfulness level with the avoidance and yield coping styles(r=-0.466,p < 0.01;r=-0.580,p < 0.01).5.The multiple linear regression analysis of influencing factors of sleep quality in colorectal cancer with enterostomy patients during chemotherapyThe results of multiple linear regression analysis showed that the family per capita monthly income, enterostomy complications occurring or not,the enterostomy type, the frequency of chemotherapy, the level of mindfulness and different coping styles were the main influencing factors of sleep quality in colorectal cancer with enterostomy patients during chemotherapy.6.The mediating affection of coping style between mindfulness level and sleep quality in colorectal cancer with enterostomy patients during chemotherapyThe study results showed that there were the partial mediating affection of coping style both the mindfulness levels and sleep quality in colorectal cancer with enterostomy patients during chemotherapy, which the coping style of face-to-face could explain 14.3% of the total affection, the avoiding style could explain 30.3% of the total affection, and the yielding style could explain 22.2% of the total affection.Conclusions1.The sleep quality in colorectal cancer with enterostomy patients during chemotherapy is generally poor.2.During chemotherapy, the mindfulness level of colorectal cancer with enterostomy patients is at the medium level. They are the more inclined to adopt the coping styles such as avoiding and yielding.3.The level of mindfulness and coping styles in colorectal cancer with enterostomy patients during chemotherapy is closely related to sleep quality. The higher level of mindfulness,it is the easier to adopt the positive coping style. The level of mindfulness and facing can positively predict the sleep quality, while avoiding and yielding can negatively predict sleep quality.4.The sleep quality in colorectal cancer with enterostomy patients during chemotherapy are influenced by per capita monthly income of families, enterostomy complications occurring or not,the type of enterostomy, frequency of chemotherapy, mindfulness level and coping style.5.There is the partial mediating affection of coping style both the mindfulness level and sleep quality in colorectal cancer with enterostomy patients during chemotherapy.
