| Objective:To treat patients with hashimoto’s thyroiditis(Hashimoto’s Thyroiditis,HT)(early stage)with the method of Shugan-Jianpi-Yangxue,observe the indicator change of thyroid peroxidase antibodies(TPo-Ab)and thyroid globulin antibodies(Tg-Ab).At the same time,the content of white cell interleukin-6(Inter Leukin-6)and tumor necrosis factor-α(TNF-α)was monitored in the patient’s blood,and the therapeutic effect of traditional prescription on HT was discussed from the perspective of cytokineology,and the immune cytokine level insight was provided for the medical treatment HT for the motherland.Methods:Clinically 40 cases were diagnosed as Hashimoto’s thyroiditis(a seizure has not been damaged),and patients with hepatic depression and spleen were identified,and were divided into 20 cases in each group of control group and treatment group by random control method.The control group mainly focused on health education and lifestyle intervention,dynamic observation,the treatment group have Shugan-Jianpi-Yangxue medicine,control group,treatment group treatment lasted 3 months.To observe the thyroid function(TSH,FT3,FT4),cytokine(IL-6,TNF-α)level,thyroid autoantibody(TPo-Ab,Tg-Ab)titer level,and compare the chinese medicine certificate score of two groups before and after the course of treatment,and monitor the patient safety index.The data index was combined and processed statistically by the statistical software SPSS24.0.Results:Baseline statistical analysis of the basic data of two groups of patients was carried out in the early stage of the study,indicating that the P value was greater than 0.05,and the study was statistically significant.(1)There was no significant change in the average amount of thyroid hormone(FT3,FT4,TSH)in two groups before and after the course of treatment(P>0.05),and there was no significant difference in thyroid hormone between groups before and after the course of treatment(P>0.05).The method of Shugan-Jianpi-Yangxue does not affect thyroid hormone levels in early patients of HT.(2)The level of TGAb about treatment group before the course of treatment was:387.07 ±102.96,after the course of treatment TGAb level was:304.12±42.81;the level of TGAb about control group before the course of treatment was:352.81 ± 88.51,after the course of treatment TGAb level was:375.00±84.69;The method of Shugan-Jianpi-Yangxue can effectively reduce TGAb level(P<0.01)in early HT patients.The level of TPOAb about treatment group before the course of treatment was:862.35(707.19,1300.00),after the course of treatment TPOAb level was:410.47(260.22,593.94);the level of TPOAb about control group before the course of treatment was:828.64(697.48,1187.30),after the course of treatment TPOAb level was:948.80(806.71,1191.42).The method of Shugan-Jianpi-Yangxue can effectively reduce TPOAb level(P<0.01)in early HT patients.(3)The level of IL-6 about treatment group before the course of treatment was:9.32±2.32,after the course of treatment IL-6 level was:5.73±0.97;the level of IL-6 about control group before the course of treatment was:8.48±2.30,after the course of treatment IL-6 level was:9.63±2.28;The method of Shugan-Jianpi-Yangxue can reduce the expression of Th2 cytokine IL-6 and balance the immune response(P<0.01)in early HT patients.(4)TNF-α level:The level of TNF-α about treatment group before the course of treatment was:103.81±15.03,after the course of treatment TNF-α level was:75.77±11.18;the level of TNF-α about control group before the course of treatment was:99.53±16.33,after the course of treatment TNF-α level was:110.36±12.01:The method of Shugan-Jianpi-Yangxue can reduce the expression of Th1 cytokine TNF-α and balance the immune response(P<0.01).At the same time,the study found that there was no significant correlation between the IL-6 levels of the two groups and the thyroid autoantibodies TGAb and TPOAb(r=0.071,P=0.663;r=0.074,P=0.649)and TNF-αlevels showed significant positive correlation with thyroid autoantibodies TGAb and TPOAb(r=0.601,P<0.01:r=0.927,P<0.01)TNF-α is a Th1 cytokine,indicating that the immune response in HT patients is still dominated by Th1.(5)The overall effective rate after treatment in the treatment group was 90.0%,the overall effective rate after the control group was 15.0%,and the overall treatment effect of the treatment group was significantly better than that of the control group(P<0.01).(6)The total points of Chinese medicine certificate before treatment group is:35.05±8.53,The total points of Chinese medicine certificate after treatment group is:14.05±6.05;The total points of Chinese medicine certificate before control group is:32.15±6.08,The total points of Chinese medicine certificate after control group is:30.30±4.50;The method of Shugan-Jianpi-Yangxue can effectively improve the clinical conditioning(P<0.01)in early HT patients.Statistical of the overall efficacy of Chinese medicine certificate score,the total efficiency of the treatment group of Chinese medicine certificate treatment is 70.0%,the total efficiency of the chinese medicine certificate treatment in the control group is 25.0%,and the improvement degree of Chinese medicine certificate in the treatment group is more obvious(P<0.01).In terms of the degree of improvement of individual evidence,the treatment group,except nerd,yellowface,intestinal gas symptoms,the remaining clinical signs were significantly improved(P<0.01),the control group only improved the symptoms of abnormal changes in mood and goodness after the course of treatment(P<0.05).(7)Safety indicators:In the course of this study,40 patients before and after the treatment the blood routine,liver and kidney function examination values are within the normal range,drug patients did not appear discomfort,no cases due to adverse reactions in the middle of the withdrawal from the study.Conclusion:For patients with ethrobbal thyroiditis in early clinical times,the method of Shugan-Jianpi-Yangxue can effectively improve their clinical symptoms,signs,reduce the level of thyroid autoantibodies titer,while IL-6,TNF-αcytokine expression also decreased,indicating that the method can regulate the immune disorder in early HT patients,There was no significant effect on thyroid hormone levels.The overall effect is accurate and the effect is safe. |
