Relationship Between Serum Phospholipid Fatty Acid Profile And Inflammation In Patients With Coronary Heart Disease

Objective: In this study,the content of phospholipid fatty acid profile in human serum was measured by gas chromatography and the levels of IL-6 and SAA in serum were detected by chemiluminescence method and ELISA(enzyme-linked immunosorbent assay).The relationship between serum phospholipid fatty acid spectrum,IL-6,SAA and coronary heart disease was investigated and the correlation between serum phospholipid fatty acid profile and inflammatory factors IL-6,SAA in patients with coronary heart disease was further investigated.Methods:(1)Subjects: 181 patients(99 males and 82 females)who were admitted to the Department of Cardiology,the Second Affiliated Hospital of Dalian Medical University from December 2018 to January 2020 due to chest tightness or chest pain were selected as subjects(99 males and 82 females),aged from 44 to 76 years old,with an average age of(60.48±8.02)years old.According to the results of coronary angiography,the subjects can be divided into coronary heart disease group and control group.86 patients with coronary heart disease who met the diagnostic criteria of coronary heart disease(coronary angiography showed that the degree of stenosis of at least one coronary artery was 50% or more than 50%).Among them,there were 52 males and 34 females,with an average age of(61.50±7.60)years old.The patients with no significant stenosis or stenosis less than 50% were taken as the control group.A total of 95 cases were included,including 47 males and 48 females,with an average age of(59.55±8.30)years old.(2)Biochemical index examination: all subjects were collected cubital venous blood on an empty stomach for at least 8 hours on the second morning,and the samples were taken and sent to the laboratory of our hospital for laboratory testing.Blood routine,troponin(CTNI),creatine kinase isoenzyme(CK-MB),renal function,pro BNP,hemagglutination routine,electrolyte,liver biochemistry,total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),fasting blood glucose(FPG)and glycosylated hemoglobin were tested.(3)Fatty acid profile detection method: all selected patients collected elbow venous blood on an empty stomach for more than 8 hours the next morning,and the blood samples were sent to the Institute of Chemical Physics of the Chinese Academy of Sciences in time.All samples were comprehensively detected by gas chromatography analyzer,and 31 fatty acid indexes and other data were obtained.(4)Detection of inflammatory factors: IL-6 was detected by IL-6 kit of Roche Diagnostic products Shanghai Co.,Ltd.for automatic electrochemiluminescence immunoassay;the detection of SAA was carried out according to the instructions of ELISA kit,and the level of SAA in serum samples was detected by enzyme-linked immunosorbent assay(ELISA).The normal values of IL-6 less than 3.4 pg/ml and the normal values of SAA less than10 mg/L.The levels of serum phospholipid fatty acid profile,inflammatory factors IL-6 and SAA in coronary heart disease group and control group were analyzed,and the data were analyzed by SPSS25.0 statistical software.Results: 1.Compared with the control group,there was no significant difference in sex,age,BMI,hypertension,diabetes,fasting blood glucose,LDL-C and other general clinical data between the two groups.Compared with the control group,the smoking history,TC,CK-MB,CTNI and pro BNP in the CHD group were significantly higher and HDL-C was significantly lower.2.Compared with the control group,the ratio ofω-6/ω-3 in the CHD group was significantly higher,while the EPA/AA ratio and DHA/AA ratio were significantly lower than those in the control group.The levels of EPA,DHA and total ω-3 PUFA in the CHD group were significantly lower than those in the control group.There was no significant difference in serum phospholipid fatty acid SFA,MUFA and other PUFA(linolenic acid,eicosatrienoic acid)between the two groups.3.Compared with the control group,the levels of IL-6 and SAA in the CHD group were significantly higher than those in the control group.4.Spearman correlation analysis was used to analyze the difference between fatty acid spectrum and Gensini score in CHD group and control group.The results showed that EPA level was negatively correlated with Gensini score(r=-0.257,P <0.05),but there was no significant correlation between ω-6/ω-3 ratio,DHA level,total ω-3 PUFA level,EPA/AA ratio,DHA/AA ratio and Gensini score(P<0.05).The results of Spearman correlation analysis of IL-6,SAA and Gensini scores showed that the level of IL-6 was positively correlated with the Gensini score of coronary artery disease,and the correlation coefficient was 0.704,and the difference was statistically significant(P<0.001).There was a positive correlation between);SAA and Gensini score,and the correlation coefficient was 0.646,and the difference was statistically significant(P<0.001).5.The Spearman correlation analysis was carried out between the indexes of fatty acid spectrum and inflammatory factors IL-6 and SAA in CHD group and control group,respectively.The results showed that EPA level and EPA/AA ratio were negatively correlated with inflammatory factors IL-6 and SAA,and the difference was statistically significant(r<0,P <0.05).Conclusion: 1.In Serum phospholipid fatty acid,the levels of EPA,the levels of DHA and the levels of total ω-3 PUFA,the ratio of EPA/AA and the ratio of DHA/AA were negatively correlated with coronary heart disease,in which EPA level was negatively correlated with the severity of coronary artery disease,while total ω-6/ω-3 ratio was positively correlated with coronary heart disease.2.The levels of serum IL-6 and SAA were correlated with coronary heart disease and positively correlated with the severity of coronary heart disease.3.The level of serum EPA and the ratio of EPA/AA in patients with coronary heart disease were negatively correlated with inflammatory factors.It is suggested that the increase of EPA level and the ratio of EPA/AA in patients with coronary heart disease has anti-inflammatory effect.
