Clinical Study On Treating HBV-related Acute-on-chronic Liver Failure With Wenyang Huayu Formula Based On Inflammatory Factor Intervention

Objective: To observe the clinical effect of Wenyang Huayu formula on HBV-related acute-on-chronic liver failure(HBV-ACLF);to explore its therapeutic mechanism based on inflammatory factor intervention.Methods: Patients with HBV-ACLF were included based on inclusion criteria and then randomly divided into the treatment group and the control group according to the random number table method.There were 30 cases in each group.Both groups were given comprehensive western medical treatment,while the treatment group additionally given Wenyang Huayu Formula.The observation period lasted 8 weeks.The observation was made to see mortality,ALT,AST,TBi L,ALB,PTA index,levels of IL-6 and IL-10,and scores of TCM syndrome and MELD;therefore,the clinical efficacy of Wenyang Huayu formla on HBV-ACLF was evaluated.Results:(1)After treatment,the total effective rate of the treatment group was 65.52%,and that of the control group was 39.29%.Better curative effect and lower motality rate were achieved in the treatment group(p < 0.05).(2)The levels of ALT,AST,and TBi L in the two groups were significantly lower after treatment,and ALB and PTA indexes significantly higher.The difference was statistically significant(p < 0.05);(3)After treatment,MELD scores and TCM syndrome scores of of survival patients in the two groups were decreased significantly with statistical difference(p<0.05);After 8 weeks,the improvement of TCM syndrome scores in the treatment group was better than those in the control group(P<0.05).(4)After treatment,the IL-6 level of survival patients in the both groups were decreased significantly(p<0.05),while the IL-6 level in the treatment group was lower than that in the control group(p<0.05).The level of IL-10 of survival patients in the both groups were increased significantly(p<0.05),while the IL-10 level in the treatment group was significantly higher than that in the control group(p<0.05).Conclusion: WenYang HuaYu formula combined with western medicine can significantly improve liver function and coagulation function in HBV-ACLF patients,inhibit the release of proinflammatory factor IL-6 and promote the release of anti-inflammatory factor IL-10 so as to reduce mortality rate and improve survival rate.It is revealed that the therapeutic effect of integrated traditional Chinese and western medicine represented by warming yang and removing blood stasis is better than that of western medicine alone,and its action mechanism may be related to the reduction of inflammatory response.
