BackgroundStroke is the main cause of death and disability in China.It has a very high disability and a high mortality rate in our country.And the incidence increases every year.Cerebral ischemic stroke is a high incidence of cerebrovascular disease in the middle-aged and elderly population.Even if it is cured,there is still a recurrence rate of about 40%,a disability rate of more than 50% and a mortality rate of 10%.Every year,there are about1.5 million to 2 million new cases in China,of which cerebral ischemic stroke accounts for70% of all stroke.And people’s health is seriously threatened.Therefore,the effective and timely evaluation,diagnosis and staging of cerebral ischemic stroke are very important for the treatment and survival of patients with cerebral ischemic stroke.Diffusion Tensor Imaging(DTI)is a sensitive and accurate method for reflecting water molecules in diffuse changes in tissues.It is one of the important methods for studying pathophysiological changes at the cellular and molecular level in patients with cerebral ischemic stroke;Fractional Anisotraphy(FA)and Apparent Diffusion Coeffecient(ADC)are commonly used parameters.Arterial Spin Labeling(ASL)is a non-invasive examination method that can determine the hemodynamic changes in patients with cerebral ischemic stroke without the need for injection of contrast agents;Cerebral Blood Flow and relative Cerebral Blood Flow are commonly used parameters.The new imaging technology has always been a research hotspot for the diagnosis and application of cerebral stroke.DTI and ASL imaging can be used in the diagnosis and staging of cerebral ischemic stroke and are of great significance.However,the research results of DTI and ASL imagingtechnology applied to cerebral ischemic stroke have not been unified.PurposeTo explore the value of magnetic resonce difusion tensor imging and arterial spin labeling imaging in the diagnosis and staging of cerebral ischemic stroke.Materials and MethodsFifty patients with cerebral ischemic stroke treated in Chinese People’s Liberati on Army 83 rd Group Army Hospital were selected.31 males and 19 females were selected.Their age was 45 to 80 years,with an average age of 62.36 ± 9.79 years.There were 5 cases of cerebral ischemic stroke in the hyperacute stage(<6h),17 ca ses in the acute stage(6h to 3d),13 cases in the subacute stage(3-10d),and 15 c ases(> 11d)in the chronic stage.Inclusion criteria: 1.Have one or more clinical sy mptoms such as dizziness,headache,hemiplegia,and sensory disturbance and meet the diagnostic criteria of conventional MRI cerebral infarction;2.Head MRI shows unilateral involvement of the infarct;3.All are first-onset with clear onset time;4.There are no contraindications for MR examination,the examination process coope rates well;5.The patient or family member signs the informed consent.Exclusion criteria: 1.Combined clinical and imaging examinations with other neurological dise ases such as cerebral hemorrhage,tumors,vascular malformations;2.Relevant treat ment before MR examination;3.Images with motion artifacts(Two physicians obse rved all levels of the patient’s sequence together,and one of them observed artifact s,which was excluded).Results1.FA,ADC,and CBF in the infarct area were smaller than those in the mirror control area(P<0.05).2.The difference in FA,ADC and rCBF in each period is statistically significant(P<0.05).There was a correlation between FA,ADC and rCBF in the infarct(P<0.05).TheFA and rCBF values showed a gradual downward trend,and the ADC values showed a gradual upward trend.The changes in ADC values were particularly obvious in the acute and chronic phases.ConclusionThe parameters of DTI and ASL have their own changes in cerebral ischemic stroke.Based on the understanding of blood flow perfusion in the infarct,the diagnosis and staging of cerebral infarction can be made accordingly,providing a reliable basis for clinical treatment plans.
