Objective:To discuss the clinical effects of closed reduction and three cannulated compression screws in the treatment of femoral neck fractures,and to analyze the relevant factors affecting the complications of femoral neck fractures,so as to provide a certain reference for clinical treatment of femoral neck fractures.Mothods:A retrospective analysis of 144 patients with femoral neck fractures admitted to the orthopedic ward of our hospital from January 2013 to December 2017,including 80 males(55.6%,80/144)and 64 females(44.4%,65/144),83 cases on the left side(accounting for 57.6%,83/144),61 cases on the right side(accounting for42.4%,61/144),with an average age of 48.08±14.580 years,according to the age of44 years old,45-59 years old,Those aged 60 years or older were divided into 45 cases in the young and middle-aged group(31.3%,45/144),72 cases in the middle-aged group(50.0%,72/144),and 27 cases in the elderly group(18.75,27/144).According to the classic Garden classification of femoral neck fractures,there were 0 cases of type I,34 cases of type II(23.6%,34/144),69 cases of type III(47.9%,47/144),and41 cases of type IV(28.5%,41/144).According to the current examination of patients with fractures in our hospital,the relevant examinations must be perfected.According to the time from injury to surgery less than or equal to 5 days and greater than 5 days,they were divided into two groups,82 cases(56.9%,82/144)and 62cases(accounting 43.1%,62/144).Retrieve the patient’s medical record home page and surgical records through the medical record system of our hospital,retrieve the patient’s preoperative and postoperative imaging data according to the medical archiving and communication system(PACS),and obtain the patient’s gender.,Age,post-injury to operation time,preoperative fracture type(Garden classification),quality of reduction(first postoperative image),different configurations of internal fixation,and other 6 factors related to internal fixation failure or femoral head necrosis.The square test and Logistic regression analysis revealed the relationship between related factors and complications of femoral neck fractures.Results:All patients were followed up,minimum follow-up time was 12 months,with an average follow-up time of 34.7 months.Seven cases of femoral neck shortening occurred with a rate of 4.9%,and 7 cases of nonunion fractures occurred with a rate of 7%.In 13 cases of femoral head necrosis,the incidence was 9.0%.Analysis of related factors showed that fracture classification(?~2=7.238,p=0.027),reduction quality(?~2=6.113,p=0.047)were related to fracture nonunion,and the time from injury to surgery(?~2=6.824,p=0.009)Related to femoral head necrosis after fracture,Logistic regression analysis shows that fracture classification and reduction quality are risk factors for fracture nonunion,and the time from injury to surgery is the risk factor for femoral head necrosis.Conclusion:Studies have shown that the overall efficacy of three cannulated compression screws in the treatment of femoral neck fractures is satisfactory,and fracture classification,post-injury to operation time,and reduction quality are the risk factors affecting the complications of femoral neck fractures. |
