Diagnostic Value Of CEA,CYFRA21-1,and ProGRP For Lung Cancer And Their Relationships With Coagulation Function | Posted on:2021-03-23 | Degree:Master | Type:Thesis | Country:China | Candidate:Y Zhang | Full Text:PDF | GTID:2404330602984189 | Subject:Internal medicine | Abstract/Summary: | PDF Full Text Request | Objective: The value of tumor markers CEA,CYFRA21-1,and ProGRP in the auxiliary diagnosis of lung cancer and their relationship with coagulation function are discussed.Methods: The clinical data on a total of 221 patients with lung cancer,51 patients with benign lung diseases and 40 healthy volunteers were reviewed.Serum carcinoembryonic antigen(CEA),cytokeratin 19 fragments(CYFRA21-1),pro-gastrin-releasing peptide(ProGRP),and d-dimer(D-D),plasma fibrinogen(FIB),platelet(PLT)were collected and then tested.The correlation between CEA 、CYFRA21-1、ProGRP and coagulation function in lung cancer patients was tested.And evaluated the value of the single and combined tests in the diagnosis of lung cancer Results: 1.The levels of CEA,CYFRA21-1,ProGRP,FIB,PLT,and D-D in lung cancer group were shown to be higher than in the control group(lung benign disease group,healthy group)(p <0.05).2.In lung cancer group,the levels of six indicators were all higher in the stage Ⅲ group than in the stage(Ⅰ+Ⅱ)group(P <0.05).The ProGRP、D-D and PLT levels in(Ⅳ)group were higher than that in stage(Ⅲ)group(P <0.05),there was no statistical difference between the two groups for the other index(P>0.05).3.FIB levels were observed to have a positive association with CYFRA21-1(p<0.001);D-D was shown to be positively correlated with CEA,ProGRP,CYFRA21-1,respectively.4.The expression of CYFRA21-1 in squamous cell carcinoma was the highest(P <0.05)and the area under the ROC curve was was the largest(AUC=0.761);Among adenocarcinomas,the area under the ROC curve of CEA was the largest(AUC=0.665).In small cell carcinoma,the values of ProGRP were the highest and the area under the ROC curve was the largest(AUC=0.824).5.Among all the indexes of lung cancer,CEA exhibited the highest diagnostic sensitivity(76.82%)and specificity(97.8%).Sensitivity,negative predictive value and the area under the AUC curve(0.928)for the diagnosis of lung cancer for the combined detection of three tumor markers,were invariably higher than if the separate detection was conducted for each index.Conclusion: The expression of tumor markers and coagulation function in patients with lung cancer varied with pathological type and clinical stage.Unusually high values of CEA,CYFRA21-1 and ProGRP have their auxiliary diagnostic value in adenocarcinoma,squamous cell carcinoma and small-cell lung cancer,respectively.The correlation between tumor markers and coagulation function of lung cancer is certain.The later the stage of lung cancer,the more obvious the abnormal coagulation function exhibited.Combined detection of CEA,CYFRA21-1,ProGRP can increase the sensitivity of lung cancer diagnosis. | Keywords/Search Tags: | lung cancer, CEA, CYFRA21-1, ProGRP, coagulation function | PDF Full Text Request | Related items |
