ObjectivesThe general conditions,imaging and laboratory examination results of 115 patients receiving MHD treatment in the blood purification center of our hospital were summarized and analyzed to explore the clinical characteristics of patients receiving MHD treatment combined with anemia in CKD patients,analyze the anemia status and related risk factors,provide a basis for future clinical diagnosis and treatment,improve the prognosis of MHD patients and improve their quality of life.Methods115 patients with chronic kidney disease who received MHD treatment in the blood purification center of the fifth affiliated hospital of zhengzhou university from December 2017 to December 2019 and met the inclusion criteria were selected as the study subjects.The basic data and laboratory data of 115 patients were analyzed in cross section.According to KDIGO guidelines,Hb ≥110g/L is deemed to meet the standard,and 115 subjects are divided into Hb sub-standard group(Hb<110g//L)and Hb sub-standard group(Hb≥110g//L).Criteria of Hb,anemia degree and related risk factors were analyzed,and various indicators of patients in the two groups were analyzed by t test,χ2 test and rank sum test.Then,the above statistically significant indicators were taken as independent variables,and independent risk factors of anemia were analyzed by binary Logistic regression analysis for patients treated with MHD.ResultsAmong the 115 patients receiving MHD treatment,81(70.4%)were males and 34(29.6%)were females,with a male/female ratio of 2.38:1.The ages ranged from 18 to 80 years old,with an average age of 63.77±10.77 years old.The dialysis age ranged from 4 to 156 months,with an average dialysis age of 38.32±34.64 months.Among the 115 patients,68(59.1%)had DKD,22(19.1%)had primary glomerular disease,13(11.3%)had hypertensive renal damage,5(4.3%)had chronic interstitial nephritis,2(1.7%)had polycystic kidney disease,and 5(4.3%)had other diseases.The average Hb of 115 patients receiving MHD treatment was 106.34±19.48g/L,and a total of 51 patients reached the standard of Hb.There were 23 cases of 110g/L≤Hb≤120g/L,accounting for 20.0%.The Hb compliance rate of the center is 44.3%.There were 64 cases of Hb failing to reach the standard,and 42 cases of 90g/L≤Hb<110g/L,accounting for 36.5%.There were 22 cases with 60g/L≤Hb<90g/L,accounting for 19.1%.The differences in age,SF,DKD,Alb,PTH,Kt/V and CRP were statistically significant(P<0.05).There was no statistically significant difference in the values of dialysis age,SI,folic acid,vitamin B12,ALP,BUN,UA,K,Ca,P,TG,TC,primary glomerular disease,hypertension,renal damage,etc.Further Logistic regression analysis showed that age,SF,PTH,Kt/V and DKD were the influencing factors for Hb failure in patients receiving MHD treatment.Conclusions1.A total of 51 MHD patients in the dialysis center reached the standard of Hb,with a compliance rate of 44.3%;2.The Hb in the non-standard group was the most in the range of 90g/L-110g/L,with a total of 41 cases,accounting for 64.1%of the total number of the non-standard group.3.Age,Kt/V,SF,PTH and DKD were the independent risk factors for Hb failure in MHD patients.
