Objective: This study analyses the nurse registration system and the nurse entrance examination system at home and abroad based on literature,and it makes a comparative analysis of the knowledge points of Chinese License Examination for Nurses from 2010 to2019 with the work tasks of clinical nurses.In order to find out the problems of Chinese nurse registration and licensure examination,give some suggestions,and provide the basis for further reform work.Methods: Search the literatures related to the nurse registration and licensure examination system and contents published in the English databases of Pubmed,AMiner and Google Scholar,Chinese academic journal full-text database(CNKI),Viper Chinese journal full-text database(VIP)and Wanfang data knowledge service platform(WF)from1999 to 2019.Search the official websites of national and regional nurse registration institutions for information about the nurse registration and licensure examination system and content in the Google search engine.Import all the bibliotheads into NoteExpress3.2,and merge the database bibliotheads.After deleting the bibliotheads according to the exact matching between the author and the bibliotheads,read the title and abstract of the bibliotheads,and retain the relevant studies related to nurse registration and licensure examination.After the initial screening of the literature after full text reading,in strict accordance with the inclusion criteria for secondary screening.Then systematically sort out and analyze the reserved documents and materials according to the nurse registration and entrance examination system,and obtain the similarities and differences at home and abroad;Record a total of 120 clinical nurse tasks from 8 dimensions of nurse tasks in the Excel table,and count the knowledge points of Chinese licensure examination of nurses from 2011 to 2019 according to the number of times they appear in the work tasks of nurses in the table.Then calculate the number of knowledge points in each task dimensions of the composition of the ratio and order.Compare and analyze the results with the order of the frequency,importance and comprehensive evaluation scores of nurses' actual clinicaltasks.Conclude the deficiencies in the Chinese nurse registration and licensure examination and put forward some suggestions.Results:Include information of nurse practice registration from 40 countries and regions in the literature and materials retrieved from countries and regions where the nursing industry is well developed.18 countries and regions have established entrance examination for nurse registration,include materials of 16 countries and regions which mentioned the form of the nurse licensure examination,the type of questions,the amount of questions,the setting of the score line,the operation of the examination and the examination outline.1.Institution of nurse registration: 72.5% of countries and regions are organized and managed by specialized nursing associations or committees,and 27.5% are registered by national health departments.The health administrative department of the government of China is responsible for the nurse registration in all administrative regions;2.Admission qualifications for nurse registration: 25% of countries and regions require a bachelor's degree in nursing,55% require a diploma in nursing,and 20% require graduation from nursing schools accredited by the education or health sector.China requires the completion of the general full-time nursing professional course for more than 3years;3.Examination method of nurse registration: 45% of countries and regions by the nurse registration examination,and 55% of countries and regions completed the practice registration work through strict qualification examination for graduates.China by the nurse licensure examination;4.Renewal of nurse's licensure: 55% of countries and regions require renewal or renewal within a few years,of which 81.8% require nurses to continue their education and meet the required hours or credits.The validity period of nurse registration in China is 5years,and there is no requirement for the duration of continuing education to renew the registration.5.Nurse entrance examination form: 80% of countries and regions adopt the traditionalwritten examination,and 20% adopt the man-machine dialogue form.Chinese licensure examination of nurses is in the form of man-machine dialogue;6.The setting of nurse entrance examination questions: 71.4% of the countries and regions only used multiple choice questions for the examination,and 28.6% of the countries and regions used multiple choice questions+subjective questions for the examination.Questions of Chinese licensure examination of nurses are all multiple choice;7.The setting of question number of nurse entrance examination: the number of questions in 85.7% of the countries and regions is fixed,and the number of questions in14.3% of the countries and regions can change according to the response situation.The question number of Chinese licensure examination of nurses is fixed;8.The setting of score of nurse entrance examination: countries and regions with fixed scores account for 75%;Chinese licensure examination of nurses adopts the standard score report;9.Nurse entrance examination outline structure: 73.3% of the countries and regions have a single dimension,and 26.7% of the countries and regions design the outline from multiple dimensions.Chinese licensure examination of nurses' syllabus is designed from three dimensions;10.Operation test section: 26.7% of countries and regions have set clinical nursing operation test,and 73.3% of countries and regions only have theoretical test.There is no operation examination part in Chinese licensure examination of nurses.11.The examination sites of Chinese licensure examination of nurses are classified and sorted according to the job category of clinical nurses:A single knowledge point check:(1)Single knowledge: 28.99%;(2)Meet the basic needs of patients: 24.36%;(3)Tasks related to drug treatment: 13.27%;(4)Evaluation: 12.97%;(5)Health education: 7.73%;(6)Communication: 6.62%;(7)Guarantee the safety of patients: 3.95%;(8)Ethics/Law: 1.89%;(9)Other tasks: 0.60%.Conclusion: 1.The requirement of education of nurses in China is at a low level,and the requirements for continuing registration in terms of knowledge and skills are lacking.The mechanism of continuing education after registration is not perfect.2.The system of Chinese licensure examination of nurses is at a high level in various countries and regions,but the part of practical skills assessment is lacking.There is still a gap between the theoretical test mode and CAT.It better to optimize the test system and the amount of questions.3.There are some inconsistencies between the knowledge points of Chinese licensure examination of nurses from 2011 to 2019 and the execution frequency and importance of the actual work tasks of clinical nurses,as well as the comprehensive evaluation scores.The examination contents should adjust the examination focus according to the actual work tasks of clinical nurses.
