Objective: Adjuvant Chemotherapy(ACT)is still have controversial in stage IB lung adenocarcinoma.The purpose of this study was to investigate the proportion of different pathological components in IB stage lung adenocarcinoma and the influence of clinicopathological features on prognosis,provide reference for postoperative adjuvant chemotherapy for IB lung adenocarcinoma.Methods:157 patients with stage IB lung adenocarcinoma who underwent lung cancer surgery from January 2004 to June 2014 in the Third Clinical Medical College of Xinjiang Medical University,divided into six groups by the proportion of their pathological components,Retrospective analysis clinical pathological data including age,gender,smoking history,pathological type,recurrence,pleural infiltration,and lymphatic infusion,and the survival curve was drawn by Kaplan-Meier survival method.LogRank test compares survival differences;multivariate COX regression analysis of risk factors affecting prognosis.Results: Univariate analysis showed that there was no significant difference in age,gender,lymph node infiltration and pleural infiltration in patients with different pathological subtypes of lung adenocarcinoma(P>0.05).The smoking history,recurrence and metastasis,and adjuvant chemotherapy have significant differences(P<0.05).The 5-year survival rate and the 5-year disease free survival have significant differences(P<0.05),and the Solid component predomianant group had the worst prognosis.The total OS is 64.3%,suggesting that solid-based and solid components may have adverse effects on recurrence and metastasis of patients,and that patients with ACT with adverse components have better DFS and OS than those without ACT.Multivariate analysis showed that pathological type,adjuvant chemotherapy and recurrence were independent risk factors for the prognosis of patients.Conclusion:The pathological subtypes of patients with stage IB are associated with prognosis,and patients who contain solid components have a worse prognosis than patients without solid components.These patients need to be more active in reviewing and treatment.And micro-nipple-based lung adenocarcinoma,consider feasible postoperative adjuvant chemotherapy.
