Objective:To investigate the value of DTI combined with neuroelectrophysiological monitoring in acoustic neuroma operation,Data and methods: Select 01,2019-01,2020 in xinjiang medical university first affiliated hospital neurosurgery hospital clinic of 38 patients with acoustic neuroma,preoperative routine with 3.0 T magnetic resonance(NMR),19 of DTI technology routine scan imaging track location of facial nerve fiber bundles and intraoperative electrophysiological monitoring validation for the experimental group use,19 cases were no DTI scan imaging technology and intraoperative electrical physiological monitoring technique in the control group,only observed in the preoperative application of DTI technology on nerve trajectory tracking and apply validation self-agreement intraoperative neural electrophysiological monitoring,The tumor resection rate,facial nerve function prognosis and postoperative complications were compared between the two groups.Results:38 cases of this study AN patients,19 cases in the experimental group of 17 cases(89.5%)completed by preoperative DTI direction to track the location of the facial nerve,and intraoperative observation,electrophysiological facial nerve corresponds to verify the actual location,17 cases of patients,intraoperative found facial nerve abdomen below 9 people,belly of 2people,abdomen above 2 people,1 upper pole,three people at a lower level;The total tumor resection rate of the experimental group was 94.7%(18/19),and that of the control group was 84.2%(16/19).After the evaluation of facial nerve function,the retention rate of facial nerve function was 84.2% in the experimental group and 63.2% in the control group.Facial nerve function retention rate 60 to 90 days after surgery: 94.7% in the experimental group and 78.9% in the control group.After analysis by SPSS statisticalsoftware,the postoperative mass resection rate and facial nerve function recovery of patients in the experimental group of the control group were significantly improved compared with the control group after treatment.Conclusion:DTI has high accuracy and good application value in the trajectory of nerve position before the surgical treatment of AN patients.Combined with intraoperative neuroelectrophysiological monitoring can greatly improve the postoperative quality of life of patients,and the prognosis is obvious. |
