Efficacy Analysis Of Partial Resection Of Puborectal Muscle In The Treatment Of Puborectalis Syndrome

Objective: To investigate the short-term and long-term clinical efficacy and safety of Sacrococcygeal ligament 6-point puborectal muscle resection in the treatment of outlet obstruction constipation caused by hypertrophy puborectalis muscle syndrome.Methods: The clinical data of 60 patients with constipation in the Department of anorectal constipation in the first affiliated Hospital of Xinjiang Medical University from December 2015 to December2018 were collected.These patients were clearly diagnosed as hypertrophy puborectalis syndromeleading to outlet obstruction constipation,and these patients were admitted to hospital.All of them were treated with 6 point puborectal muscle resection via sacrococcygeal region.Through preoperative and 1、3、6、12months after surgery,the patients were measured by anorectal manometry,defecography and subjective score scale for the anal resting pressure,anal rectal angle during forced defecation,anal canal maximum systolic pressure,the time of defecations,Laborious defecation,rectal resting pressure,the number of defecations,the length of high pressure belt of the anal canal,Wexner score and detected relevant indicators.These were used for data collation and case analysis and comparison.Results: In this case analysis,57cases(95%)were effective.After Sacrococcygeal ligament 6-point puborectal muscle resection of the sacrococcygeal region,the measured anal resting pressure,rectal and anal reflex threshold,the time of defecations,anal pressure band length,Laborious defecation,Wexner total score,rectal and rectal initial sensory threshold were all reduced.The anorectal Angle at rest,the number of defecation,and the anorectal Angle in the force discharge period all increased(P<0.05).There was no significant difference in anal canal maximum systolicpressure,incomplete defecation 1 month after the surgery,rectal resting pressure(P>0.05).Conclusion: Sacrococcygeal ligament 6-point puborectal muscle resection is effective in treating outlet obstruction constipation,which can reduce the length of the anal canal high pressure belt,reduce pathological anal canal pressure,and relieve constipation symptoms of outlet obstruction.It is worth further clinical research and implementation.
