Endoscopic Transsphenoidal Surgery For Growth Hormone Pituitary Tumor Prognostic Factors

Objective:Through the study of the effect of transsphenoidal surgery in the treatment of growth hormone pituitary adenoma,to study the related factors and postoperative complications,to evaluate the effect of surgery and guide the clinical treatment.Methods:From December 2014 to December 2019,the clinical data of 102 cases of growth hormone pituitary tumor treated by neuroendoscopy in neurosurgery of Xinjiang Medical University were reviewed,including the level of growth hormone before operation,tumor size,gender,age,pituitary function,postoperative pathological results and Ki-67,etc.Through the long-term follow-up,through the analysis of these data,to explore the related factors and postoperative complications that affect the level of growth hormone.Results:Among the 102 cases of GH pituitary adenoma in this study,the remission rate of GH was 80%,10-20ug/L,71%,and more than 20ug/L,23%respectively.The difference between the 10-20ug/L and 20ug/L groups was statistically significant(P<0.05).The remission rates of microadenoma,macroadenoma and macroadenoma were 85%,47.5%and 0%,respectively There was statistical significance in the remission rate of growth hormone(P<0.05).In the tumor classification,the remission rate of 0-2 grade was 64%,the remission rate of 3-4 grade was 36%,the difference was statistically significant(P<0.05).The remission rate of Ki-67?2%was 54%,the remission rate of>3%was 53%,the difference was not statistically significant(P>0.05).There were 18 patients with CSF rhinorrhea and 7 patients with intracranial infection There were 11 patients with transient diabetes insipidus,15 patients with hypopituitarism,3 patients with epistaxis,and no death.Conclusion:Endoscopic transsphenoidal surgery is an effective and safe method for the treatment of growth hormone pituitary adenoma.The level of growth hormone after surgery is related to knosp classification,preoperative growth hormone,tumor diameter,but not to age,gender,Ki-67.The higher the level of knosp,the more likely the postoperative complications will be.
