| Objective:The subject mainly through the effect of Zeji liquid extract nano materials on the tuberculous wound formed by in the later stage of Scrofula to evaluate the level of wound healing,to study its curative effect on the tuberculous wound,and to reveal its mechanism by cytological experiments.Methods:From March 2019 to September 2019,61 patients with tuberculous wound who met the diagnostic criteria of traditional Chinese and western medicine were randomly divided into and two groups.On the basis of oral same antituberculous drugs,the experimental group was added with Zeji liquid extract nano materials and control group was added with medical Vaseline gauze.The interval of dressing change was 2 days,the course of treatment is 28 days.On the 1st day,7th day,15th day and 28th day,tissues(1 mm×1 mm×1mm)were obtained in the same position of the wound focus and the skin from the wound margin of the patients in the two groups.The changes of macrophages were observed under transmission electron microscope,the comprehensive curative effect was observed by color ultrasound or magnetic resonance(MRI),and the safety index of peripheral blood was monitored.In addition,The expression of NLRP3,Caspase-1,IL-1p,IL-18 and IL-33 genes in the model group(BCG-group),the experimental group(Zeji liquid extract nano materials group)and the control group(CK group)were determined by cytological test.The expression of IL-1β and IL-18 in the tuberculous wound tissue was observed by immunohistochemistry before and after treatment.Results:1.The total effective rate of the clinical experimental group was 96.77%,the total effective rate of the control group was 63.33%,the difference was statistically significant(P<0.05),the effect of the experimental group was significantly better than that of the control group.2.After treatment,the syndrome score of the clinical experimental group and the control group decreased significantly(P<0.05),the decrease in the experimental group is larger than that in the control group(P<0.01).3.The results of transmission electron microscope showed that the morphology of macrophages in the clinical experimental group was better than that in the control group.4.In experimental group of cytology experiment,the expression of NLRP3 and IL-18 was significantly decreased afer treated with Zeji liquid extract nano materials(P<0.01),while Caspase-1,IL-1 p and IL-33 were still at a high level(P<0.01).5.The results of immunohistochemistry on the 1st and 28th day in the clinical experimental group confirmed that IL-1β and IL-18 in the experimental group were down regulated(P<0.01).Conclusion:Zeji liquid extract nano materials can make the functions of traditional Chinese medicine,such as removing blood stasis and putrefaction,detoxification and muscle production,play a full role through the slow release and degradation of nanomaterials,and obviously promote the healing of tuberculous wound.NLRP3,IL-1p,IL-18 can be down regulated obviously,which can regulate and induce the growth and differentiation of macrophages,and is one of the healing mechanism of tuberculous wound. |
