ObjectiveThrough for the comparison on the clinical therapeutic effect and economic evaluation between laparoscopic myomectomy combined with magnetic resonance imaging and simple laparoscopic myomectomy to search the clinical value of magnetic resonance imaging in laparoscopic myomectomy and provide patients with a more appropriate selection before operation.MethodsA total of 176 patients with hysteromyoma were treated by laparoscopic myomectomy in Affiliated Hospital of TaiShan Medical University from July 2014 to July 2016.All the cases confirmed to be hysteromyoma postoperatively by pathology were divided into 2 groups according to patient’s preferances,including observation group(112 cases)with MRI-guided laparoscopic myomectomy,control group(63 cases)with laparoscopic myomectomy.We collect the patient’s general clinical date and surgical treatment,collect the date of patient’s menstruation,gynecological examination,transvaginal ultrasonography in 3 months and 6 months after surgery by outpatient appointments.Health economics evaluation is included in the statistical analysis.Results1.Preoperative date between the observation group and control group:Between the two groups of age,BMI,pregnant time,produce time,hemoglobin,P>0.05,there were no statistically difference.2.Perioperative date between the observation group and control group:Between the two groups of minimum diameter,maximum diameter,operation time,blood loss,exhaust time,hospital stay time,P>0.05,there were no statistically difference;But the difference is statistically significant on hystermyoma number in operation,P<0.05.3.Follow up date between the observation group and control group:After 3 months,it showed that the hystermyoma residual rate(9.80%)in observation group was significantly lower than the control group(20.63%),and the difference between the two groups was statistically significant(P<0.05).After 6 months,it showed that the hystermyoma recurrent rate(8.04%)in control group was similar with the observation group(7.94%),and the difference between the two groups was not statistically significant(P<0.05).4.Health economic evaluation between the observation group and control group:Between the two groups of direct medical costs,direct non-medical costs,indirect costs and total costs,P>0.05,there were no statistically difference.ConclusionThe patients who have clinical symptoms requiring surgical treatment,can be excised the hysteromyoma precisely during operation on the basis of MRI.This way can help reduce the residual rate of hysteromyoma that hiding in the deep myometrium.On the surface,MRI examination before operation increases the patient’s medical expenses,but from the point of view of health economics,it does not increase the patient’s medical expenses.Whether it is for the treatment from the patient’s point of view,or from the social point of view for disease medical decision,MRI can be used as a simple and effective preoperative examination selection that should be widely used in clinical.
